腰大池引流术引起中枢感染的危险因素研究

摘　　要：目的:探讨腰大池引流术引起中枢感染的危险因素。方法:选取2016年1月—2021年12月上海市第十人民医院崇明分院收治的368例行腰大池引流手术患者作为研究对象,分析其一般情况、引流情况、术后管理以及腰大池引流引起中枢感染情况。结果:368例腰大池引流术患者中,18例患者脑脊液培养阳性,感染比例4.89%。其中引流置管时间、是否存在引流管夹闭、引流管周围皮肤是否存在脑脊液外渗、是否每日更换无菌敷贴感染组与未感染组比较,差异有统计学意义(t=4.92;χ^(2)=10.629、13.397、4.273,P<0.05)。多因素logistic回归分析提示是否存在引流管夹闭,腰大池置管引流管时间以及引流管周围皮肤是否存在脑脊液外渗是腰大池引流引起中枢感染的影响因素(OR=3.463、0.083、3.463,P<0.05)。结论:引流管夹闭,腰大池置管引流管时间以及引流管周围皮肤是否存在脑脊液外渗是腰大池引流引起中枢感染的危险因素。Objective:To investigate the risk factors for central infection by lumbar pool drainage.Methods:368 patients admitted to the hospital for lumbar pool drainage surgery from January 2016 to December 2021 were selected as the study population.Their general condition,drainage,postoperative management,and central infection due to lumbar pool drainage were retrospectively analyzed.Results:Of 368 patients with lumbar pool drainage,18 patients had positive cerebrospinal fluid cultures,representing 4.89% of infections.There were statistically significant differences in the time of drainage placement,presence of drainage tube clamping,presence of cerebrospinal fluid extravasation from the skin around the drainage tube,and whether the sterile dressing was changed daily between the two groups(t=4.92,χ^(2)=10.629,13.397,4.273,P<0.05).Multifactorial logistic regression analysis suggested that the presence of drainage tube entrapment,the duration of lumbar pool placement drainage tube and the presence of cerebrospinal fluid extravasation from the skin around the drainage tube were influential factors for central infection due to lumbar pool drainage(OR=3.463,0.083,3.463,P<0.05).Conclusion:Drainage tube clamping,duration of lumbar pool placement drainage tube and the presence of cerebrospinal fluid extravasation from the skin around the drainage tube are risk factors for central infection due to lumbar pool drainage.
