摘 要:目的:评价超声特征联合血清甲状旁腺激素(iPTH)在原发性甲状旁腺功能亢进症(PHPT)诊断中的临床价值。方法:回顾性分析2013年3月—2020年4月石河子大学医学院第一附属医院收治的120例行甲状腺或甲状旁腺切除手术的患者病历资料,以病理结果为金标准,结合其超声特征和iPTH结果,逐一进行单因素分析,筛选出差异有统计学意义的变量建立多因素回归模型。绘制受试者工作特征曲线(ROC),判断超声特征、iPTH以及回归模型在PHPT中的诊断效能。结果:经前进法logistic回归分析发现,甲状旁腺的体积和iPTH水平是影响PHPT发生的独立危险因素,差异有统计学意义(P<0.05)。logistic回归模型对PHPT预测的准确率为89.2%,ROC曲线下面积为0.95。结论:甲状旁腺体积与iPTH在诊断原发性甲状旁腺功能亢进中有重要临床意义,超声特征联合iPTH可提高对PHPT的诊断价值。Objective:To evaluate the clinical value of ultrasound features combined with serum parathyroid hormone(iPTH)in the diagnosis of primary hyperparathyroidism(PHPT).Methods:The data of 120 patients who underwent thyroidectomy or parathyroidectomy in the sample hospitals from March 2013 to April 2020 were retrospectively analyzed,and the pathological findings were used as the gold standard,combined with their ultrasound characteristics and iPTH results,and univariate analysis was performed one by one to screen out variables with statistically significant differences to establish a multifactorial regression model.Subject work characteristic curves(ROC)were plotted to determine the diagnostic efficacy of ultrasound features,iPTH,and regression models in PHPT.Results:A forward logistic regression analysis revealed that parathyroid in volume and iPTH level were independent risk factors for the development of PHPT,with statistically significant differences(P<0.05).Logistic regression model had an accuracy of 89.2% for PHPT prediction and an area under the ROC curve of 0.95.Conclusion:Parathyroid volume and iPTH are clinically important in the diagnosis of primary hyperparathyroidism,and ultrasound features combined with iPTH can improve the diagnostic value of PHPT.
