摘　　要：目的探讨人乳头状瘤病毒(HPV)感染宫颈病变组织中E2F转录因子1(E2F1)、磷酸化蛋白激酶B(p-Akt)、微小核糖核酸-424(miR-424)与血清环氧合酶-2(COX-2)信使核糖核酸(mRNA)水平.方法选取2018年6月-2022年9月浙江省丽水市人民医院收治的190例HPV感染宫颈病变患者为研究对象,根据疾病进展情况分为慢性宫颈炎(CCS)组(58例),宫颈上皮内瘤变(CIN)组(81例),宫颈癌组(51例);比较三组患者、CIN组不同分级及不同HPV感染状态下组织中E2F1、P-Akt、miR-424与血清COX-2 mRNA水平,并采用Spearman相关性分析E2F1、P-Akt、miR-424、COX-2 mRNA与HPV感染的相关性.结果宫颈癌组患者组织中E2F1阳性率、P-Akt表达率及血清COX-2 mRNA水平均高于CIN组和CCS组,CIN组高于CCS组;宫颈癌组患者组织中miR-424均低于CIN组、CCS组,CIN组低于CCS组(P<0.05);CINⅢ级组患者组织中P-Akt表达率及血清COX-2 mRNA水平均高于CINⅡ级组、CINⅠ级组,CINⅡ级组高于CINⅠ级组;CINⅢ级组患者组织中miR-424水平均低于CINⅡ级组、CINⅠ级组,CINⅡ级组低于CINⅠ级组(P<0.05);HPV阳性患者组织中E2F1阳性率、P-Akt表达率及血清COX-2 mRNA水平均高于HPV阴性患者;HPV阳性患者组织中miR-424水平均低于HPV阴性患者(P<0.05);E2F1、P-Akt、COX-2 mRNA与HPV感染呈正相关(P<0.05);miR-424与HPV感染呈负相关(P<0.05).结论E2F1、P-Akt、miR-424及血清COX-2 mRNA与宫颈病变的进程密切相关,且其与HPV感染具有密切的关联,对HPV感染具有一定指示作用.OBJECTIVE To investigate the expression levels and clinical significance of E2F transcription factor 1(E2F1),phosphorylated protein kinase B(p-Akt),microribonucleic acid-424(miR-424)and serum cyclooxygenase-2(COX-2)messenger ribonucleic acid(mRNA)in human papillomavirus(HPV)-infected cervical lesions.METHODS Totally 19O patients with cervical lesions infected by HPV admitted to the People's Hospital of Lishui City,Zhejiang Province from Jun.2018 to Sep.22 were selected as the study subjects,and were divided into the chronic cervicitis(CCS)group(58 cases),the cervical intraepithelial neoplasia(CIN)group(81 cases)and the cervical cancer(CC)group(5l cases)according to the progress of the disease.The levels of E2Fl,P-Akt,miR-424 and serum COX-2 mRNA in the tissues of the three groups of patients,the CIN group with different grades.and different HPV infection status were compared,and the correlation between E2F1,P-Akt,miR-424,COX-2 mRNA and HPV infection were analyzed by Spearman correlation analysis method.RESULTS The positive rate of E2F1,the expression rate of P-Akt and the level of serum COX-2 mRNA of the CC group were higher than those of the CIN group and the CCS group,and that of the CIN group was higher than that of the CCS group.The level of miR-424 in the CC group was lower than that in the CIN group and the CCS group,and that in the CIN group was lower than that in the CCS group(P<0.05).The expression rate of P-Akt and level of serum COX-2 mRNA in tissue of the CIN gradeⅢgroup were higher than that of the CIN gradeⅡgroup and the CIN gradeⅠgroup,and that of the CIN gradeⅡgroup was higher than that of the CIN gradeⅠgroup.The level of miR-424 in the CINⅡgroup was lower than that in the CINⅡgroup and the CINⅠgroup,and that in the CINⅡgroup was lower than that in the CINⅠgroup(P<0.05).The positive rate of E2F1,the expression rate of P-Akt and the level of serum COX-2 mRNA in HPV-positive patients were higher than those in HPV-negative patients.The level of miR-424 in tissues of HPV-positive p
