摘  要:骨关节炎(OA)主要导致中老年人关节疼痛、僵硬和功能障碍,发病机制尚不明确。环状RNA(circRNA)在OA患者中差异表达,并参与多基因、多靶点的调控网络。circRNA通过炎症反应联合增殖凋亡、自噬分化、氧化应激和机械应力等生物学过程参与OA,其中绝大多数circRNA在软骨细胞、终板软骨细胞、间充质干细胞、滑膜细胞和巨噬细胞等细胞中充当微RNA分子海绵的作用,进而促进OA的病理进展。也存在少部分circRNA在细胞内经蛋白质结合、甲基化修饰等方式,或经细胞间的微囊泡和外泌体机制参与OA的发病过程。通过对上述方向研究将为开发创新的生物标志物和治疗策略提供深入的理论基础,并为OA的个体化治疗和预防提供新的科学依据。Osteoarthritis(OA)primarily causes joint pain,stiffness,and functional impairment in middle-aged and elderly individuals,and its pathogenesis remains unclear.Circular RNA(circRNA)expresses differentially in OA patients and participates in a regulatory network involving multiple genes and targets.circRNA participates in OA through inflammation combined with multiple biological processes,including proliferation,apoptosis,autophagy,differentiation,oxidative stress and mechanical stress.The majority of circRNA function as molecular sponges for microRNA in chondrocytes,endplate chondrocytes,mesenchymal stem cells,synovial cells,and macrophages,thereby promoting the pathological progression of OA.Some circRNAs are involved in the pathogenesis of OA either by intracellular binding to proteins and methylation modification or via intercellular microvesicular and exosomal mechanisms.In-depth exploration of the aforementioned directions will provide a solid theoretical basis for the development of innovative biomarkers and treatment strategies,as well as new scientific evidence for individualized treatment and prevention of OA.
