摘 要:机械通气是公认治疗急性呼吸衰竭的方法,然而其会导致呼吸机相关肺损伤(VILI),增加患者肺不张、肺炎的发生率,影响患者预后,加重医疗负担。因此应采取有效的肺保护通气策略,减少肺损伤的发生。根据VILI发生的不同机制,可采用不同的肺保护策略,如抑制炎症反应、适宜的通气方法、减少高浓度氧对肺的损伤。目前有关减轻VILI较多为肺保护性通气方面的研究,相关新的药物和光生物调节治疗等方法报道较少。未来应在新药物研发和生物学治疗方面进行深入研究,以为VILI患者的治疗提供新策略。Mechanical ventilation is recognized as a treatment for acute respiratory failure,however,it can cause ventilation-induced lung injury(VILI)and increase the incidence of atelectasis and pneumonia in patients,thus affecting the prognosis of patients and increasing the medical burden.Therefore,effective lung protection strategies should be adopted to reduce the occurrence of lung injury.According to the different mechanisms of VILI,different lung protection methods can be used,such as inhibiting inflammation,adopting appropriate ventilation strategies,and reducing lung damage caused by hyperoxia.At present,the research on reducing VILI is mainly about lung protective ventilation,and there are few reports on new drugs and photobiological regulation treatment.In the future,it is necessary to increase the research on new drug development and biological treatment,so as to provide new strategies for the treatment of VILI.
