中医康复辅以炒茴香热敷法对肾癌患者生命质量及自我效能的影响

摘　　要：目的:探讨中医康复辅以炒茴香热敷法对肾癌患者生命质量及自我效能的影响。方法:将根治性肾切除手术的局限性肾癌患者218例根据随机信封1∶1抽签原则分为热敷组与对照组各104例。患者术后均予常规护理,对照组加中医康复护理,热敷组在对照组基础上予炒茴香热敷法,14天后评定两组患者生命质量及自我效能评分。结果:热敷组患者术后首次排气排便、腹胀消失、拔除尿管与住院时间均少于对照组(P<0.05);热敷组康复期间应激性溃疡、出血、切口感染、静脉血栓、肺部感染等并发症发生率为2.9%(3/104),低于对照组的17.3%(18/104)(P<0.05);两组患者康复后自我效能评分高于康复前(P<0.05),热敷组高于对照组(P<0.05);康复后热敷组患者躯体、角色、情绪、认知及社会功能等生命质量评分高于对照组(P<0.05),疲劳、恶心呕吐、疼痛、失眠、便秘等生命质量评分低于对照组(P<0.05)。结论:中医康复辅以炒茴香热敷法能从整体上提高肾癌术后患者生命质量及自我效能,减少并发症,促进患者康复。Objective:To discuss the influence of TCM rehabilitation supplemented with hot compress with fried fennel on quality of life and self-efficacy of the patients with renal cancer.Methods:All 218 patients with limited renal cancer undergoing radical nephrectomy were divided into the hot compress group and the control group with 104 cases in each group according to random envelope 1:1 lottery principle.All the patients accepted routine care after the surgery,and the control group conventional care of TCM rehabilitation,and the hot compress group hot compress with fried fennel on the foundation of the therapy the control group received,to assess the scores of quality of life and self-efficiency between both groups in 14 days.Results:Time of first exhaust time and defecation time after the surgery,abdominal distension disappearance time,catheter removal time and hospital stay in the hot compress group were less than those in the control group(P<0.05);the incidence of the complications including stress ulcer,bleeding,incision infection,venous thrombosis and pulmonary infection of hot compress group during the recovery period was 2.9%(3/104),lower than 17.3%(18/104)of the control group(P<0.05);self-efficiency scores of both groups after the rehabilitation were higher than these before the rehabilitation(P<0.05),hot compress group was higher than the control group(P<0.05);the scores of quality of life such as somatic,role,emotional,cognitive and social functioning of hot compress group after the recovery were higher than these of the control group(P<0.05);the scores of quality of life covering fatigue,nausea and vomiting,pain,insomnia and constipation were lower than these of the control group(P<0.05).Conclusion:TCM rehabilitation supplemented with hot compress with fried fennel could improve quality of life and self-efficacy of the patients with renal cancer as a whole and reduce the complications so as to promote the patients'recovery.
