心电图QRS波时限及QTc与急性左心衰竭患者院内临床转归的相关性

摘　　要：【目的】探讨心电图QRS波时限及校正QT间期(QTc)与急性左心衰竭患者院内临床转归的相关性。【方法】选取2019年5月至2022年5月榆林市中医医院收治的87例急性左心衰竭患者,根据患者住院期间预后情况将其分为预后不良组(n=30)和预后良好组(n=57)。比较两组入院时(急性左心衰竭发作后24 h内)的一般资料及氨基末端B型脑钠前体肽(NT-proBNP)、可溶性生长刺激表达基因2蛋白(sST2)、心电图相关参数[QRS波时限、校正QT间期(QTc)]、超声心动图相关参数[左室射血分数(LVEF)和左室舒张末期内径(LVEDD)]。采用多因素Logistic回归分析影响急性左心衰竭患者院内临床转归的相关因素。【结果】预后不良组NT-proBNP、sST2高于预后良好组,差异有统计学意义(P<0.05)。预后不良组QRS波时限、QTc长于预后良好组(P<0.05),LVEF低于预后良好组(P<0.05),LVEDD与预后良好组比较,差异无统计学意义(P>0.05)。Logistic回归分析显示,NT-proBNP、sST2、QRS波时限、QTc是影响急性左心衰竭患者院内临床转归的相关因素(P<0.05)。【结论】急性左心衰竭患者QRS波时限、QTc明显延长,且是影响患者院内临床转归不良的相关因素。【Objective】To explore the correlation between the QRS wave duration and corrected QT interval(QTc)on electrocardiogram and the hospital clinical outcomes of patients with acute left heart failure.【Methods】A total of 87 patients with acute left heart failure admitted to the Yulin Traditional Chinese Medicine Hospital from May 2019 to May 2022 were selected.They were divided into a poor prognosis group(n=30)and a good prognosis group(n=57)according to their in-hospital prognosis.The general information,N-terminal pro-B-type natriuretic peptide(NT-proBNP),soluble growth stimulation gene 2 protein(sST2),electrocardiogram parameters[QRS wave duration,corrected QT interval(QTc)],echocardiographic parameters[left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD)],and multi-factor Logistic regression analysis were used to analyze the factors that affect the hospital clinical outcomes of patients with acute left heart failure.【Results】The levels of NT-proBNP and sST2 in the poor prognosis group were higher than those in the good prognosis group,and the difference was statistically significant(P<0.05).The QRS wave duration and QTc in the poor prognosis group were longer than those in the good prognosis group(P<0.05),while the LVEF was lower than that in the good prognosis group(P<0.05),and there was no significant difference in LVEDD between the two groups(P>0.05).Logistic regression analysis showed that NT-proBNP,sST2,QRS wave duration,and QTc were factors that affected the hospital clinical outcomes of patients with acute left heart failure(P<0.05).【Conclusion】The QRS wave duration and QTc on electrocardiogram are significantly prolonged in patients with acute left heart failure,and they are related factors affecting the hospital clinical outcomes of these patients.
