摘  要:血管性认知障碍(VCI)是一种由多种原因引发的异质性疾病。VCI的发生与潜在的血管疾病有关,而皮质下缺血性脑血管病(SIVD)已成为VCI中的一种主要同质亚型。SIVD起病隐匿,且临床表现与阿尔茨海默病存在重叠,因此临床鉴别较困难,极易漏诊和误诊。脑脊液中存在多种生物标志物,可更及时、准确地反映脑组织的病理生理改变,从而辅助VCI-SIVD早期诊断与鉴别诊断,对于提高VCI-SIVD早期疗效具有重要临床意义。因此,全面了解VCI-SIVD脑脊液生物标志物可以为疾病的诊疗提供新思路。Vascular cognitive impairment(VCI)is a heterogeneous disease caused by a variety of causes.The occurrence of VCI is related to underlying vascular diseases,and subcortical ischemic vessel disease(SIVD)has become a major homogeneous subtype of VCI.The onset of SIVD is insidious,and the clinical manifestations overlap with Alzheimer′s disease,so it is difficult to distinguish SIVD from Alzheimer′s disease,and it is easy to be missed and misdiagnosed.There are a variety of biomarkers in cerebrospinal fluid,which can reflect the pathophysiological changes of brain tissue more timely and accurately,help to assist the early diagnosis and differential diagnosis of VCI-SIVD,and have great clinical significance for improving the early curative effect of VCI-SIVD.Therefore,a comprehensive understanding of the biochemical markers in cerebrospinal fluid of VCI-SIVD can provide new ideas for the diagnosis and treatment of the disease.
