摘 要:目的探讨短时受精周期中减数分裂Ⅰ(MⅠ)期卵子过夜受精对体外成熟、受精和胚胎发育的影响。方法选择2018年9月至2022年6月在广州市番禺区妇幼保健院生殖医学科进行治疗的117对不孕不育患者夫妇117个短时体外受精周期,共1532枚卵子。根据随机数字表法分为过夜受精组(54例,54个周期)和常规培养组(63例,63个周期)。常规培养组将MⅠ期卵子置于卵裂培养皿中过夜培养,过夜受精组在颗粒细胞剥除后将MⅠ期卵子重新置于原受精皿中过夜受精。比较两组MⅠ期卵子的原核(PN)及胚胎发育情况,包括成熟率、受精率、卵裂率、可利用胚胎率、优质胚胎率、囊胚形成率。同时对比过夜受精组内MⅠ期、第一极体(1PB)和第二极体(2PB)卵子间以上各项结果参数的差异。结果过夜受精组MⅠ期卵子成熟率、受精率显著高于常规培养组[75.51%(74/98)比54.35%(50/92)、32.65%(32/98)比19.57%(18/92)](P<0.01),两组MⅠ期卵子2PN率、1PN率、多PN率、0PN率、2PN卵裂率、可利用胚胎率、优质胚胎率和囊胚形成率比较差异无统计学意义(P>0.05)。过夜受精组内MⅠ期、1PB和2PB卵子的受精率、2PN率、2PN卵裂率、可利用胚胎率比较差异有统计学意义(P<0.01)。其中,2PB卵子的受精率、2PN率高于1PB[94.85%(516/544)比76.79%(86/112)、71.69%(390/544)比55.36%(62/112)](P<0.05),MⅠ期受精率、2PN率低于1PB(P<0.05),MⅠ期受精率、2PN率低于2PB(P<0.05);MⅠ期2PN卵裂率低于1PB[87.50%(14/16)比96.77%(60/62)](P<0.05);2PB可利用胚胎率高于1PB[89.45%(424/474)比76.92%(60/78)](P<0.05),MⅠ期可利用胚胎率低于2PB(P<0.05)。结论颗粒细胞剥除后,将MⅠ期卵子重新置于原受精皿中过夜受精能够有效改善MⅠ期卵子的成熟、受精情况,对改善胚胎情况也有一定帮助。Objective To discuss the effect of overnightfertilization of meiotic stageⅠ(MⅠ)oocytes in short-term fertilization cycle on in vitro maturation,fertilization and embryonic development.Methods A total of 117 couples of infertile patients who were treated in the Department of Reproductive Medicine,Panyu Maternal and Child Care Service Center of Guangzhou from Sep.2018 to Jun.2022 were included for 117 short-time in vitro fertilization cycles,with a total of 1532 oocytes.They were divided into an overnight fertilization group(54 cases,54 cycles)and a routine culture group(63 cases,63 cycles)according to random number table method.In the routine culture group,the stage MⅠeggs were placed in the cleavage Petri dish for overnight culture,and in the overnight fertilization group,the stage M I eggs were placed in the original fertilization dish again for overnight fertilization after the removal of granulosa cells.The pronuclear(PN)and embryonic development of MⅠovum were compared between the two groups,including maturation rate,fertilization rate,cleavage rate,available embryo rate,high-quality embryo rate and blastocyst formation rate.At the same time,the differences of the above parameters between the MⅠ,the first polar body(1PB)and the second polar body(2PB)oocytes in the overnight fertilization group were compared.Results The maturation rate and fertilization rate of MⅠovum in the overnight fertilization group were significantly higher than those in the routine culture group[75.51%(74/98)vs 54.35%(50/92),32.65%(32/98)vs 19.57%(18/92)](P<0.01).There was no statistically significant difference in 2PN rate,1PN rate,poly PN rate,0PN rate,2PN cleavage rate,available embryo rate,good embryo rate and blastocyst formation rate between the two groups(P>0.05).The fertilization rate,2PN rate,2PN cleavage rate and available embryo rate of MⅠstage,1PB and 2PB eggs in the overnight fertilization group were statistically significantly different(P<0.01).The fertilization rate and 2PN rate of 2PB eggs were higher than
