摘  要:目的分析母血及新生儿脐血维生素K水平及其影响因素,提高临床对新生儿维生素K缺乏的重视。方法随机选取2019年6月1日至2020年3月31日于首都医科大学附属北京妇产医院定期产检并分娩的100名孕产妇,采用高效液相色谱法(HPLC)检测母血及脐血维生素K水平。结果100名单胎妊娠孕产妇的血维生素K水平为0.001~4.270μg/L,平均[0.203(0.130,0.328)]μg/L,100名新生儿脐血中维生素K水平为0~2.033μg/L,平均[0.014(0.007,0.036)]μg/L,其中低于0.2μg/L界值者57名。母血与脐血的维生素K平均浓度比值为26.14∶1。不同年龄、文化程度、工作性质、居住现状、孕周情况、新生儿出生体质量母血以及不同年龄、工作性质、新生儿出生体质量脐血维生素K水平比较差异无统计学意义(P>0.05)。不同文化程度脐血维生素K水平比较差异有统计学意义(P<0.05),城镇居住组脐血维生素K水平高于农村居住组[0.016(0.010,0.040)μg/L比0.008(0.001,0.023)μg/L](P<0.05),足月产组脐血维生素K水平高于早产组[0.017(0.009,0.040)μg/L比0.005(0.001,0.017)μg/L](P<0.05)。将产妇年龄、文化程度、工作性质、居住现状、孕周情况以及新生儿出生体质量纳入多重线性回归分析,结果显示,孕周、工作性质及新生儿出生体质量是脐血维生素K水平的影响因素(P<0.05或P<0.01)。结论早产儿、孕母文化程度较低及农村地区的新生儿脐血维生素K水平较低,应重点关注早产儿、低出生体质量儿、孕母文化水平较低及农村地区的新生儿,积极预防维生素K缺乏导致的出血性疾病,改善母儿结局。Objective To analyze the level of vitamin K in maternal blood and umbilical cord blood and its influencing factors,and to raise clinical attention to neonatal vitamin K deficiency.Methods A total of 100 pregnant and lying-in women who were regularly examined and delivered at Beijing Obstetrics and Gynecology Hospital,Capital Medical University from Jun.1,2019 to Mar.31,2020 were randomly selected,and the levels of vitamin K in maternal and umbilical blood were determined by high performance liquid chromatography(HPLC).Results The blood vitamin K level of 100 pregnant and lying-in women with fetal pregnancy was 0.001-4.270μg/L,with an average of[0.203(0.130,0.328)]μg/L,and that of 100 newborns was 0-2.033μg/L,with an average of[0.014(0.007,0.036)]μg/L,of which 57 were below the 0.2μg/L threshold.The average concentration ratio of vitamin K in maternal and umbilical blood was 26.14∶1.There was no significant difference in vitamin K levels in maternal blood of different age,education level,nature of work,living status,gestational age,newborn birth weight and umbilical blood of different age,nature of work,newborn birth weight(P>0.05).There was statistically significant difference in umbilical cord blood vitamin K level between different education levels(P<0.05).The level of vitamin K in umbilical blood of urban living group was higher than that of rural living group[0.016(0.010,0.040)μg/L vs 0.008(0.001,0.023)μg/L](P<0.05).The level of vitamin K in cord blood in the full-term group was higher than that in the preterm group[0.017(0.009,0.040)μg/L vs 0.005(0.001,0.017)μg/L](P<0.05).The maternal age,education level,work nature,living status,gestational weeks and newborn birth weight were included in the multiple linear regression analysis.The results showed that gestational weeks,work nature and newborn birth weight were the influencing factors of vitamin K level in umbilical cord blood(P<0.05 or P<0.01).Conclusion The level of vitamin K in umbilical cord blood is low in premature infants,pregnant mothers wi
