自体血胸膜固定术治疗难治性继发性自发性气胸的系统评价

摘　　要：目的系统评价自体血胸膜固定术治疗难治性继发性自发性气胸的疗效与安全性。方法通过检索数据库,纳入自建库至2023年3月的文献,文献地区、发表时间无限制。由2名研究者独立筛选文献,提取资料并评价纳入研究的偏倚风险后,采用Stata 16.0软件进行meta分析。结果自体血胸膜固定术的总有效率:81%,95%CI(0.74,0.87),使用50 ml血量进行胸膜固定的有效率:81%,95%CI(0.68,0.91)。不良反应率:9%,95%CI(0.02,0.2)。发热发生率:4%,95%CI(0.01,0.08)。结论现有证据显示,自体血胸膜固定治疗难治性继发性气胸的疗效显著,安全性较高。由于纳入研究数量和质量的限制,上述结论尚需更多高质量的随机试验予以验证。Objective To systematically evaluate the efficacy and safety of autologous haemopleural pleurodesis in the treatment of refractory secondary spontaneous pneumothorax.Methods The database was searched to include literature from the time of its creation to March 2023,with no restrictions on the region of literature or time of publication.Metaanalysis was performed using Stata 16.0 software after two researchers independently screened the literature,extracted information and evaluated the risk of bias of the included studies.Results The overall efficacy of autologous blood pleural pleurodesis were 81%,95%CI(0.74,0.87).The efficacy of pleural pleurodesis using 50 ml blood volume was 81%,95%CI(0.68,0.91).The adverse reaction rate was 9%,95%CI(0.02,0.2).The pyrexia rate was 4%,95%CI(0.01,0.08).Conclusion The available evidence suggests that autologous haemopleural pleurodesis is highly effective and safe in the treatment of refractory secondary pneumothorax.However,due to limitations in the number and quality of included studies,these findings need to be validated by additional high-quality randomized trials.
