摘  要:目的:探讨血清移动抑制因子(MIF)、25-羟维生素D_(3)(25-(OH)D_(3))、Toll样受体2(TLR2)mRNA水平动态监测在活动性肺结核患者疾病转归评估中的应用价值。方法:选取我院2019年3月-2021年5月收治且完成随访的118例活动性肺结核患者为研究对象,治疗6个月后随访6个月,根据疾病复发情况将患者分为复发组(17例)和未复发组(101例)。比较两组治疗前、治疗2、4、6个月后血清MIF、25-(OH)D_(3)、TLR2mRNA水平,分析血清MIF、25-(OH)D_(3)、TLR2mRNA水平与疾病转归相关性,偏回归分析影响肺结核患者疾病转归的危险因素,比较不同高低水平复发风险度。结果:复发组治疗2、4、6个月后血清MIF、TLR2mRNA水平高于未复发组,25-(OH)D_(3)水平低于未复发组(P<0.05);Spearman分析,治疗2、4、6个月后血清MIF、TLR2mRNA水平与疾病复发呈正相关,血清25-(OH)D_(3)水平与疾病复发呈负相关(P<0.05);Logistic分析显示,血清MIF、TLR2 mRNA、25-(OH)D_(3)水平为活动性肺结核患者疾病复发的独立危险因素(P<0.05);治疗后血清MIF、TLR2 mRNA高水平、血清25-(OH)D_(3)低水平的复发风险较高(P<0.05)。结论:动态监测血清MIF、25-(OH)D_(3)、TLR2mRNA水平变化对预测活动性肺结核患者疾病转归的状况存在指导效果。Objective:To explore the value of dynamic monitoring of serum mobility inhibitory factor(MIF),25 hydroxyvitamin D_(3)(25-(OH)D_(3))and toll like receptor 2(TLR2)mRNA levels in the evaluation of disease outcome in patients with active pulmonary tuberculosis.Methods:118 patients with active pulmonary tuberculosis who were admitted to our hospital from March 2019 to may 2021 and completed the follow-up were selected as the study subjects.After 6 months of treatment,the patients were followed up for 6 months.According to the disease recurrence,the patients were divided into the relapse group(17 cases)and the non relapse group(101 cases).The levels of serum MIF,25-(OH)D_(3)and TLR2 mRNA were compared between the two groups before treatment,2,4 and 6 months after treatment,and the correlation between the levels of serum MIF,25-(OH)D_(3)and TLR2 mRNA and the disease outcome was analyzed.The risk factors affecting the disease outcome of patients with pulmonary tuberculosis were analyzed by partial regression,and the recurrence risk of different levels was compared.Results:After 2,4 and 6 months of treatment,the levels of serum MIF and TLR2 mRNA in the relapsed group were higher than those in the non relapsed group,and the levels of 25-(OH)D_(3)were lower than those in the non relapsed group(P<0.05);Spearman analysis showed that serum MIF and TLR2 mRNA levels were positively correlated with disease recurrence after 2,4 and 6 months of treatment,and serum 25-(OH)D_(3)levels were negatively correlated with disease recurrence(P<0.05);Logistic analysis showed that serum MIF,TLR2 mRNA and 25-(OH)D_(3)levels were independent risk factors for disease recurrence in patients with active pulmonary tuberculosis(P<0.05);The high level of serum MIF and TLR2 mRNA and the low level of serum 25-(OH)D_(3)after treatment had a higher risk of recurrence(P<0.05).Conclusion:Dynamic monitoring of serum MIF,25-(OH)D_(3)and TLR2 mRNA levels has a guiding effect on predicting the disease outcome of patients with active pulmonary tuberculosis.
