乙肝五项、前S1抗原与HBV-DNA联合检测的临床应用

摘　　要：目的:探讨乙肝五项、前S1抗原和HBV-DNA在肝炎诊断的价值;方法:收集上海市松江区中心医院2022年7月1日-8月31日检测乙肝五项的标本3364例,分析其乙肝五项、前S1抗原的检出率、HBV-DNA的载量,另外收集上海市松江区中心医院2022年3月1日-2022年8月31日HBV-DNA高载量标本46例,中载量标本54例,低载量标本76例,对照组标本52例,统计并分析其ALT、AST、GGT含量;结果:3364例标本中HBSAg阳性率为9.3%,检出率较高的模式分别为全阴、25(+)、2(+)、245(+)、145(+)、5(+);在主要的乙肝五项型别中,只有145(+)和135(+)型别检测到HBV-DNA,135(+)检出率高于145(+);前S1抗原与HBeAg检出率不同;不同DNA载量的四个组别ALT、AST、GGT差别均有统计学意义,四个组别ALT、AST、GGT不完全相同;结论:乙肝五项、前S1抗原和HBV-DNA联合检测对乙肝的诊断有重要的意义。Objective:Explore the value of hepatitis B serologic test、Pre-S1 antigen and HBV-DNA in the diagnosis of hepatitis B.Methods:Collect 3364 samples for hepatitis B serologic test from July 1st,2022 to August 31st,2022 in Songjiang Central Hospital,then analyse the detection rate of hepatitis B serologic test、Pre-S1 antigen and the load of HBV-DNA.In addition,collect 46 samples with high load of HBV-DNA,54 samples with medium load,76 samples with low load and 52 samples of normal control group from March 1st,2022 to August 31st,2022 in Songjiang Central Hospital,then detect and make statistical analysis on the concentration of ALT、AST、GGT in these samples.Results:The positive rate of HBSAg in 3364 samples was 9.3%.The types with high positive rates were total negative,25(+),2(+),245(+),145(+),5(+)respectively;Among the main types of hepatitis B serologic test,HBV-DNA was only detected in 145(+)and 135(+),while the dection rate of 135(+)was higher than 145(+).The detection rate of Pre-S1 antigen was different from HBeAg.In the four groups with different DNA load,the difference among the concentration of ALT、AST、GGT were of statistical significance.The four groups were not identical.Conclusion:The combined application of hepatitis B serologic test、Pre-S1 antigen and HBV-DNA is of great significance in the diagnosis of hepatitis B.
