摘 要:目的 了解南宁市新报告HIV感染者首次CD4^(+)T淋巴细胞(简称CD4^(+)T细胞)数检测结果和合并梅毒感染情况,为艾滋病防控提供科学依据。方法 对2020—2022年在南宁市疾病预防控制中心进行HIV确证的新报告HIV感染者并完成首次CD4^(+)T细胞检测的病例,使用ELISA和TRUST方法进行梅毒抗体检测,收集性别、年龄、婚姻、民族和文化程度等人口学特征和行为学特征,采用χ^(2)检验对首次CD4^(+)T细胞检测结果和HIV合并梅毒感染进行分析,采用二分类Logistics回归模型对晚发现病例进行单因素和多因素分析。结果 共有1 503例新报告HIV感染者纳入分析,其中男1 112例(73.99%),平均年龄52.43岁,已婚792例(52.69%),壮族986例(65.60%),初中及以下文化1 213例(80.71%),异性性传播1 322例(87.96%)。合并梅毒共118例(7.85%),不同年龄、婚姻状况、文化程度、传播途径等合并梅毒感染差异有统计学意义(P<0.05)。CD4^(+)T细胞数的均值为(270.36±226.96)个/μL,不同年龄、文化程度和传播途径对CD4^(+)T细胞数检测的均值显示,≥50岁年龄段、初中及以下文化水平、异性性传播首次检测CD4^(+)T淋巴细胞计数均值较低,差异有统计学意义(P<0.05);在不同性别、年龄、文化程度和传播途径的检测对象,CD4^(+)T淋巴细胞检测构成比结果,男性、≥50岁年龄段、初中及以下文化水平、异性性传播人群首次检测CD4^(+)T淋巴细胞计数<200个/μL构成比较高,差异有统计学意义(P<0.05)。晚发现病例(CD4^(+)T细胞计数<200个/μL)占比最高,为650例(43.25%),多因素Logistics回归分析显示,男性(OR=1.599,95%CI:1.253~2.040),30~49岁(OR=2.205,95%CI:1.378~3.527),≥50岁(OR=2.640,95%CI:1.633~4.268)是晚发现的危险因素。结论 南宁市新报告HIV感染者,晚发现病例占比较高,合并梅毒感染率较高,应该扩大HIV检测,及早发现感染者,同时进行梅毒的检测和监测。Objective To understand the results of the first CD4^(+)T lymphocytes test and Treponemia pallidum(TP) co-infection among patients with human immunodeficiency virus in Nanning,so as to provide references for the effective prevention and treatment of HIV patients.Methods HIV-infected persons who were first confirmed and accomplished the first CD4^(+)T lymphocytes test in Nanning Center for Disease Control and Prevention from January 2020 to December 2022 were collected and analyzed,ELISA and TRUST methods to detect TP antibody,and demographic and behavioral characteristics,such as sex,age,marriage,nationality,and education level,were collected.The results of the first CD4^(+)T lymphocytes and HIV and TP co-infection were analyzed by chi-square test,and unviariate and multivariate analyses were performed using the binary Logistic regression model to the late detection cases.Results A total of 1 503 patients with newly diagnosed HIV infection were included in this study,including 1 112(73.99%) males,with an average age of 52.43 years.792(52.69%) that were married,986(65.60%) that were Zhuang nationality,1 213(80.71%) with education of junior high school or below and 1 322(87.96%) that were transmitted through heterosexual contact.The co-infection rate of TP was 118(7.85%).There were significant differences in TP co-infection rates among different ages,marriage,education level and route of transmission(P<0.05).The mean CD4^(+)T lymphocyte count was(270.36± 226.96) cells/μL.In terms of the mean value of CD4^(+)T lymphocyte count detected by different age,education level and route of transmission,the mean value of the first CD4^(+)T lymphocyte count was lower in those patients aged ≥ 50 years,with educational level of junior high school and lower and heterosexual transmission,and the difference was statistically significant(P<0.05).In terms of the test subjects of different gender,age,education level and route of transmission,there were significant differences in the mean CD4^(+) lymphocyte count(200 cells/μL) and
