早发不典型症状的儿童皮肌炎1例

摘　　要：患儿女,4岁,臀部暗紫红斑1个月,面部、甲周红斑20 d。皮肤科情况:双侧臀部片状暗紫红斑,面颊部皮肤可见对称性片状红斑;双手甲周皮肤暗红斑、色素沉着斑,可见毛细血管扩张;双手近端指间关节伸侧可见Gottron丘疹。皮损组织病理示:表皮网篮状角化过度,基底层细胞空泡变性。真皮浅层血管周围淋巴细胞灶性浸润,可见色素失禁。诊断:儿童皮肌炎。A 4-year-old female child had dark purple erythema on buttocks for one month and erythema on face and perionychium for twenty days.Dermatological examination showed flaky dark purple erythema on buttocks,symmetrical flaky erythemas on the skin of the cheeks.The dark erythematous hyperpigmentation spots and telangiectasias skin were around the nails.The Gottron papules were on the extensor side of the proximal interphalangeal joints of both hands.Pathology of the skin lesions showed the basket-like hyperkeratosis in the epidermis.The vacuolar degeneration of the basal layer cells were detected in the epidermis.The focal infiltration of perivascular lymphocytes in the superficial dermis and pigment incontinence were present.The diagnosis was juvenile dermatomyositis.
