广西某农村地区中老年艾滋病感染状况及已婚者性行为特征调查

摘　　要：目的了解广西某农村地区中老年艾滋病感染状况及已婚者性行为特征,为老年人艾滋病防治提供依据。方法统计分析全国艾滋病防治信息系统中某农村地区报告的50岁及以上病例,并随机抽取该地区1209名50岁及以上已婚有配偶的调查对象,采用自行设计的问卷开展一对一调查。结果广西某农村地区50岁及以上中老年人群艾滋病报告感染率1.13%(158/13983),79.11%(125/158)的报告病例婚姻状况为已婚有配偶,双阳家庭病例58例。家庭内双阳病例中,男性病例主要为非婚异性性接触感染(96.55%,28/29),女性病例的配偶/固定性伴阳性接触史高于男性病例(χ2=30.750,P<0.001)。1209名调查对象平均年龄(66.83±8.73)岁,最近1次与配偶/固定性伴发生性行为使用安全套占12.58%(140/1113),84.01%(935/1113)人与配偶/固定性伴发生性行为从未使用安全套,多因素分析结果显示,男性安全套使用频率高于女性(OR=1.760,95%CI:1.256~2.465),50~59岁年龄组是70岁以上年龄组的3.208倍(OR=3.208,95%CI:2.147~4.793),艾滋病知识不知晓者(OR=2.548,95%CI:1.709~3.799)更容易发生无保护性行为。2.89%(35/1209)调查对象有婚外性行为。82.86%(29/35)婚外性伴侣非本村村民,男性婚外性伴数、有偿性行为比例均比女性高且差异有统计学意义(χ2=9.058,P<0.05;χ2=5.600,P<0.05)。结论非婚商业异性性接触是农村地区中老年男性的主要感染途径,配偶/固定性伴阳性是女性的主要感染途径,无保护性行为增加了该人群艾滋病感染的风险。应加强农村地区中老年人群艾滋病的宣传教育,提高HIV检测的普及性,减少不安全性行为的发生。Objective To understand the status and influencing factors of AIDS marital infection among middle-aged and older people in rural areas of Guangxi and to provide evidence for the prevention and treatment of AIDS among the elderly.Methods Data were downloaded from the national AIDS comprehensive information management system,and analysis was conducted on the reported HIV/AIDS cases aged 50 and above in Guangxi.A special survey was carried out among married couples aged 50 and above randomly selected from a rural areas in Guangxi.A self-designed questionnaire was used to conduct a one-to-one survey.Results The HIV infection rate reported by middle-aged and older people in rural area of Guangxi was 1.13%(158/13983).Of the reported cases,79.11%(125/158)were married,and 58 cases were from double-positive families.Among the HIV-seroconcordant couples,the male cases were mainly infected by non-marital heterosexual contact(96.55%,28/29),and the female cases had a higher rate of having a spouse/fixed partner who was HIV-positive than the male cases(χ2=30.750,P<0.000).The average age of 1209 respondents was(66.83±8.73)years.The condom use rate was 12.58%(140/1113)in the latest sexual behavior with a spouse/fixed partner,and 84.01%(935/1113)of the elderly never used condoms.The results of multivariate analysis showed that the frequency of condom use in men was higher than that in women(OR=1.760,95%CI:1.256-2.465),in the age group of 50-59 years old was 3.21 times higher than that of the age group of 70 years old(OR=3.21,95%CI:2.15-4.79),and those without knowledge of AIDS were more likely to have unprotected sex(OR=2.55,95%CI:1.71-3.80).2.89%(35/1209)of the respondents had extramarital sex,with 82.86%(29/35)of the extramarital sexual partners not being from the local village.The number of extramarital sexual partners and the proportion of paid sexual behaviors of middle-aged and older men were higher than those of women,and the difference was statistically significant(χ2=9.058,P<0.05;χ2=5.600,P<0.05).Conclusion Commercia
