自拟中药汤剂联合舍曲林治疗肝气郁结型抑郁症的临床效果

摘　　要：目的探究自拟中药汤剂联合舍曲林治疗肝气郁结型抑郁症的临床效果。方法选取我院2020年4月至2022年12月收治的116例肝气郁结型抑郁症患者,采用随机数字表法将其分为对照组和联合组,各58例。对照组给予舍曲林治疗,联合组在对照组基础上给予自拟中药汤剂治疗。比较两组的治疗效果。结果治疗4、6、8周后,联合组的汉密尔顿抑郁量表17项(HAMD-17)、汉密尔顿焦虑量表(HAMA)评分低于对照组(P<0.05)。联合组的治疗总有效率显著高于对照组(P<0.05)。治疗8周后,联合组的烦躁、善忘、精神抑郁、疲乏无力积分低于对照组(P<0.05)。治疗4、6、8周后,联合组的自动思维问卷(ATQ)评分低于对照组(P<0.05)。治疗8周后,联合组的恶心呕吐、口干、便秘评分低于对照组(P<0.05)。结论自拟中药汤剂联合舍曲林治疗肝气郁结型抑郁症患者的效果显著,可降低中医证候积分,改善抑郁、焦虑情绪及负性自动思维,减轻不良反应。Objective To explore the clinical effect of self-made traditional Chinese medicine decoction combined with sertraline in the treatment of depression of liver Qi stagnation type.Methods A total of 116 patients with depression of liver Qi stagnation type admitted in our hospital from April 2020 to December 2022 were selected and divided into control group and combined group by random number table method,with 58 cases in each group.The control group was treated with sertraline,and the combined group was treated with self-made traditional Chinese medicine decoction on the basis of the control group.The therapeutic effects of the two groups were compared.Results After 4,6 and 8 weeks of treatment,the scores of Hamilton Depression Scale-17 Items(HAMD-17)and Hamilton Anxiety Scale(HAMA)in the combined group were lower than those in the control group(P<0.05).The total effective rate of treatment in the combined group was significantly higher than that in the control group(P<0.05).After 8 weeks of treatment,the scores of irritability,amnesia,mental depression and fatigue in the combined group were lower than those in the control group(P<0.05).After 4,6 and 8 weeks of treatment,the Automatic Thoughts Questionnaire(ATQ)score of the combined group was lower than that of the control group(P<0.05).After 8 weeks of treatment,the scores of nausea and vomiting,dry mouth and constipation in the combined group were lower than those in the control group(P<0.05).Conclusion The self-made traditional Chinese medicine decoction combined with sertraline in the treatment of patients with depression of liver Qi stagnation type has a significant effect,which can reduce the score of TCM syndromes,improve depression,anxiety and negative automatic thinking,and reduce adverse reactions.
