天智颗粒治疗血管性痴呆的有效性及安全性Meta分析

摘　　要：目的采用Meta分析评价天智颗粒治疗血管性痴呆的有效性及安全性。方法对中国知网(CNKI)、维普中文科技期刊数据库(VIP)、万方医学网、Cochrane图书馆、PubMed数据库等进行“主题词+自由词”检索,时间截止至2023年2月。由2名评价员选择符合纳入及排除标准的文献,筛选并评价文献质量,收集并整理相关数据。运用RevMan5.4软件进行Meta分析。结果共纳入22篇文献,2679例患者,其中试验组1499例,对照组1180例。合并结果显示,试验组的治疗总有效率高于对照组(P<0.00001)。天智颗粒的治疗总有效率高于空白对照(P<0.0001);天智颗粒联合奥拉西坦的治疗总有效率高于奥拉西坦(P=0.0001);天智颗粒联合多奈哌齐的治疗总有效率高于多奈哌齐(P<0.0001);天智颗粒与阿米三嗪萝巴新片的治疗总有效率比较,差异无统计学意义(P=0.84);天智颗粒的治疗总有效率高于吡拉西坦及其他促智药(P<0.00001)。所有纳入文献均未报道严重不良反应。结论天智颗粒能够一定程度上改善血管性痴呆患者的认知功能,且用药安全性较好。Objective To evaluate the efficacy and safety of Tianzhi granules in the treatment of vascular dementia by Meta-analysis.Methods The"subject words+free words"were searched in China National Knowledge Infrastructure(CNKI),China Science and Technology Journal Database(VIP),Wanfang Medical Network,Cochrane Library and PubMed database until February 2023.Two evaluators selected the literature that met the inclusion and exclusion criteria,screened and evaluated the quality of the literature,and collected and collated relevant data.Meta-analysis was performed using RevMan5.4 software.Results A total of 22 articles and 2679 patients were included,including 1499 cases in the experimental group and 1180 cases in the control group.The combined results showed that the total effective rate of treatment in the experimental group was higher than that in the control group(P<0.00001).The total effective rate of the treatment of Tianzhi granule was higher than that of blank control(P<0.0001);the total effective rate of the treatment of Tianzhi granule combined with oxiracetam was higher than that of oxiracetam(P=0.0001);the total effective rate of the treatment of Tianzhi granule combined with donepezil was higher than that of donepezil(P<0.0001);there was no significant difference in the total effective rate of treatment between Tianzhi granules and almitrine robaxin tablets(P=0.84);the total effective rate of the treatment of Tianzhi granule was higher than that of piracetam and other intelligence-promoting drugs(P<0.00001).No serious adverse reactions were reported in all the included literatures.Conclusion Tianzhi granule can improve the cognitive function of patients with vascular dementia to a certain extent,and the drug safety is good.
