双歧杆菌三联活菌联合四联疗法治疗老年幽门螺杆菌相关性胃溃疡患者的效果

摘　　要：目的:观察双歧杆菌三联活菌联合四联疗法治疗老年幽门螺杆菌(Hp)相关性胃溃疡患者的效果。方法:选取2020年1月至2023年1月该院收治的125例老年Hp相关性胃溃疡患者进行前瞻性研究,根据随机数字表法将其分为对照组62例和观察组63例。对照组采用四联疗法治疗,观察组在对照组基础上联合双歧杆菌三联活菌胶囊治疗,比较两组临床疗效、Hp根除率、治疗前后胃黏膜组织学评分、肠道菌群组成和不良反应发生率。结果:观察组治疗总有效率为95.24%(60/63),高于对照组的80.65%(50/62),Hp根除率为80.95%(51/63),高于对照组的62.90%(39/62),差异均有统计学意义(P<0.05);治疗后,两组炎性细胞浸润、胃黏膜厚度及炎症活动性等胃黏膜组织学评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,观察组乳酸杆菌、双歧杆菌菌落数均高于对照组,肠球菌、肠杆菌、酵母菌菌落数均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:双歧杆菌三联活菌胶囊联合四联疗法治疗老年Hp相关性胃溃疡患者可提高治疗总有效率和Hp根除率,改善肠道菌群组成,降低胃黏膜组织学评分,效果优于单纯四联疗法治疗。Objective:To observe effects of Bifidobacterium triple viable combined with quadruple therapy in treatment of elderly patients with Helicobacter pylori(Hp)-associated gastric ulcer.Methods:A prospective study was conducted on 125 elderly patients with Hp-related gastric ulcer admitted to this hospital from January 2020 to January 2023.According to the random number table method,they were divided into control group(62 cases)and observation group(63 cases).The control group was treated with quadruple therapy,while the observation group was treated with Bifidobacterium triple viable capsules on the basis of that of the control group.The clinical efficacy,the Hp eradication rate,the gastric mucosal histological score before and after the treatment,the intestinal flora conditions,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of treatment in the observation group was 95.24%(60/63),which was higher than 80.65%(50/62)in the control group;the eradication rate of Hp was 80.95%(51/63),which was higher than that of the control group(62.90%,39/62);and the differences were statistically significant(P<0.05).After the treatment,the histological scores of gastric mucosa such as inflammatory cell infiltration,gastric mucosal thickness and inflammatory activity in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the number of Lactobacillus and Bifidobacterium colonies in the observation group was higher than that in the control group,the number of Enterococcus,Enterobacter and yeast colonies was lower than that in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Bifid triple viable capsules combined with quadruple therapy in the treatment of the elderly patient
