超声测量子宫弓状动脉血流参数在前置胎盘合并胎盘植入诊断中的价值

摘  要:目的:观察超声测量子宫弓状动脉血流参数在前置胎盘合并胎盘植入诊断中的价值。方法:回顾性分析2021年4月至2022年12月该院收治的140例前置胎盘孕妇的临床资料,按手术病理检查结果将其分为胎盘植入组(n=62)和非胎盘植入组(n=78),比较两组子宫弓状动脉血流参数水平和不同胎盘植入程度的前置胎盘孕妇子宫弓状动脉血流参数水平,采用受试者工作特征曲线(ROC)评估子宫弓状动脉血流参数在前置胎盘合并胎盘植入诊断中的价值。结果:胎盘植入组收缩期峰值流速高于非胎盘植入组,搏动指数、血管阻力指数低于非胎盘植入组,差异均有统计学意义(P<0.05);ROC曲线分析结果显示,收缩期峰值流速、搏动指数、血管阻力指数诊断前置胎盘合并胎盘植入的曲线下面积(AUC)分别为0.776、0.596、0.632;植入型前置胎盘收缩期峰值流速高于粘连型前置胎盘,差异有统计学意义(P<0.05);ROC曲线分析结果显示,收缩期峰值流速诊断前置胎盘合并胎盘植入程度的AUC为0.774。结论:超声测量子宫弓状动脉收缩期峰值流速诊断前置胎盘合并胎盘植入中具有一定的价值。Objective:To observe value of ultrasonic measurement of uterine arcuate artery blood flow parameters in diagnosis of placenta previa complicated with placenta implantation.Methods:The clinical data of 140 pregnant women with placenta previa admitted to this hospital from April 2021 to December 2022 were retrospectively analyzed.According to the results of surgical and pathological examination,these pregnant women were divided into placental implantation group(n=62)and non-placental implantation group(n=78).The blood flow parameters of uterine arcuate artery and the blood flow parameters of uterine arcuate artery in the pregnant women with placenta previa with different degrees of placenta implantation were compared between the two groups.The receiver operating curve(ROC)was used to evaluate the value of uterine arcuate artery blood flow parameters in the diagnosis of placenta previa combined with placenta implantation.Results:The peak systolic velocity of the placenta implantation group was higher than that of the non-placenta implantation group,the pulsatility index and the vascular resistance index were lower than those of the non-placenta implantation group,and the differences were statistically significant(P<0.05).The ROC curve showed that the AUCs of peak systolic velocity,pulsatility index and vascular resistance index in the diagnosis of placenta previa with placenta implantation were 0.776,0.596 and 0.632,separately.The peak systolic velocity of implanted placenta previa was higher than that of adhesive placenta previa,and the difference was statistically significant(P<0.05).The ROC curve results showed that the AUC of peak systolic velocity in the diagnosis of placenta previa complicated with placenta implantation was 0.774.Conclusions:The ultrasound measurement of uterine arcuate artery peak systolic velocity has a certain value in the diagnosis of placenta previa complicated with placenta implantation.
