封闭负压引流技术联合大隐静脉腔内激光闭合手术在高原地区下肢静脉曲张性溃疡患者中应用效果分析

摘　　要：目的:探讨封闭负压引流技术(VSD)联合大隐静脉腔内激光闭合手术在高原地区下肢静脉曲张性溃疡患者中的应用效果。方法:选取2021年8月—2022年7月日喀则市定日县卫生服务中心收治的下肢静脉曲张性溃疡患者40例作为研究对象,采用随机数字表法分为激光组与传统组,各20例。激光组行VSD联合大隐静脉腔内激光闭合手术,传统组行VSD联合大隐静脉抽剥手术。比较两组治疗效果。结果:两组术后均无感染发生,随访期内均无溃疡复发。激光组术后溃疡面积小于传统组,术后创面愈合时间、术后肉芽组织形成时间早于传统组,术后换药次数少于传统组,差异有统计学意义(P<0.05)。结论:VSD联合大隐静脉腔内激光闭合术在高原地区下肢静脉曲张性溃疡患者中的应用效果显著,可以缩小术后溃疡面积,缩短愈合时间,减少换药次数。Objective:To explore the application effect of vacuum sealing drainage(VSD)combined with endovascular laser closure of great saphenous veins in patients with varicose ulcer of lower extremity in high plateau area.Methods:A total of 40 patients with varicose ulcer of lower extremity admitted to Shigatse Dingri County Health Service Center from August 2021 to July 2022 were selected as the study subjects.They were divided into laser group and traditional group according to random number table method,with 20 cases in each group.The laser group underwent VSD combined with endovascular laser closure of great saphenous veins,and the traditional group underwent VSD combined with great saphenous vein stripping.The treatment effects were compared between the two groups.Results:There was no postoperative infection in the two groups,and there was no ulcer recurrence during the follow-up period.The postoperative ulcer area in the laser group was smaller than that in the traditional group,the postoperative wound healing time and postoperative granulation tissue formation time in the laser group were earlier than those in the traditional group,and the number of postoperative dressing changes in the laser group was less than that in the traditional group,and the difference was statistically significant(P<0.05).Conclusion:VSD combined with endovascular laser closure of great saphenous veins has a significant application effect in patients with varicose veins of lower extremity in high plateau area,which can reduce the postoperative ulcer area,shorten the healing time,and reduce the number of dressing changes.
