摘　　要：目的:探讨双采造血干细胞及血气分析的脐血采集方法对新生儿脐血血气分析的影响。方法:选取2020年1月—2022年8月在佛山市南海区第四人民医院出生的新生儿180例作为研究对象,采用随机数字表法分为观察组与对照组,各90例。观察组同时采集造血干细胞制备用脐血和血气分析用脐血,对照组仅采集血气分析用脐血。比较两组脐血血气分析结果,观察组脐血送广东省脐带血造血干细胞库制备及检测污染情况、脐血量、冻前有核细胞计数。结果:两组脐血血气分析的酸碱度(pH)与剩余碱比较,差异无统计学意义(P>0.05)。观察组乙肝表面抗原、丙肝表面抗体、梅毒抗体、艾滋病抗体、巨细胞病毒抗体检测和细菌/霉菌污染检测结果均为阴性,脐血量为(70.87±28.07)mL,有核细胞数(TNC)为(9.85±5.29)×10^(8)个,符合广东省脐血库脐血入库要求。结论:双采脐血采集方法能在保证脐血量和脐血TNC的基础上,同时保证脐血血气分析结果的稳定性和准确性,值得脐血血气分析和脐血采集领域的关注。Objective:To investigate the effect of double collection method of umbilical cord blood for hematopoietic stem cells and blood gas analysis on neonatal umbilical cord blood gas analysis.Methods:A total of 180 neonates born in the Foshan City Nanhai District Fourth People's Hospital from January 2020 to August 2022 were selected as the study subjects.They were divided into observation group and control group according to random number table method,with 90 cases in each group.Umbilical cord blood for hematopoietic stem cell preparation and umbilical cord blood for blood gas analysis were collected simultaneously in the observation group,while umbilical cord blood for blood gas analysis alone was collected in the control group.The results of umbilical cord blood gas analysis were compared between the two groups.Umbilical cord blood in the observation group was sent to Guangdong Cord Blood Bank for preparation and detection of contamination,umbilical cord blood volume and pre-freezing total nucleated cell count.Results:There was no significant difference in the power of hydrogen(pH)and the base excess between the two groups(P>0.05).The results of hepatitis B surface antigen,hepatitis C surface antibody,syphilis antibody,AIDS antibody,cytomegalovirus antibody detection and bacterial/fungal contamination detection results were all negative,the umbilical cord blood volume was(70.87±28.07)mL,and the number of total nucleated cells(TNC)was(9.85±5.29)×10^(8)cells,which met the entry criteria of Guangdong Cord Blood Bank.Conclusion:The double collection method of umbilical cord blood can ensure the stability and accuracy of umbilical cord blood gas analysis results while ensuring the umbilical cord blood volume and TNC in umbilical cord blood,which is worthy of attention in the field of umbilical cord blood gas analysis and umbilical cord blood collection.
