海军基层军医对慢性疼痛评估及管理体会的质性研究

摘　　要：目的了解分析海军基层军医评估、管理慢性疼痛患者的经验体会,以指导提高相关医疗保障水平。方法2023年3月至6月,采取目的抽样法选取来自海军水面舰艇部队、海军陆战队及海军航空兵部队的10例基层军医进行半结构式访谈,采用Colaizzi主题分析法对访谈资料进行分析、归纳并提炼主题。结果海军基层军医对官兵慢性疼痛评估及管理的经验体会可归纳为4个主题:智能管理工具缺乏,疼痛管理相关培训不足,慢性疼痛诊疗方法单一,疼痛自我管理意义重大。结论为提高海军基层军医对于官兵慢性疼痛的保障水平,应从推进慢性疼痛智能管理工具应用、强化慢性疼痛管理培训、提供跨学科多模式诊疗、促进慢性疼痛自我管理等方面入手。Objective To understand and analyze the experience of evaluating and managing chronic pain patients by naval grassroots military doctors,in order to guide the improvement of relevant medical support levels.Methods From March to June 2023,a purposive sampling method was used to select 10 grassroots military doctors from the Navy's surface vessel unit,Marine Corps,and Navy Aviation Corps for semi-structured interviews.The interview data were analyzed,summarized,and extracted using the Colaizzi theme analysis method.Results The experience and understanding of naval grassroots military doctors in evaluating and managing chronic pain in officers and soldiers can be summarized into four themes:lack of intelligent management tools,insufficient training in pain management,single diagnosis and treatment methods for chronic pain,and significant self management of pain.Conclusion In order to improve the level of support for chronic pain among officers and soldiers by grassroots military doctors in the Navy,efforts should be made to promote the application of intelligent management tools for chronic pain,strengthen training in chronic pain management,provide interdisciplinary and multimodal diagnosis and treatment,and promote selfmanagement of chronic pain.
