摘 要:目的研究血流限制训练(blood flow restriction training,BFRT)联合神经肌肉电刺激疗法(neuromuscular electrical stimulation,NMES)在膝关节镜术后股四头肌伸肌滞后康复中的应用效果。方法选择2021年6月至2022年12月在庐山康复疗养中心行膝关节镜手术治疗的患者共计60例,按照随机数字表分为对照组和观察组各30例。对照组行常规康复治疗+NMES,观察组行常规康复治疗+NMES+BFRT。比较两组患者大腿周径、滞后关节活动度变化、股四头肌等速肌力、康复效果。结果治疗后4、6周,观察组美国特种外科医院膝关节功能评分量表(HSS)评分(78.90±3.17)分、(89.20±5.12)分明显高于同期对照组(69.63±4.69)分、(79.93±3.63)分,差异有统计学意义(P<0.05);治疗后4、6周,观察组关节活动度滞后角度明显低于同期对照组(P<0.01);治疗后4、6周,观察组股四头肌等速肌力峰力矩明显高于同期对照组(P<0.01);治疗后6周,观察组大腿周径较对照组有明显提升(P<0.01)。结论BFRT联合NMES在膝关节镜术后患者功能康复治疗中具有显著疗效,有助于患者股四头肌伸肌滞后的恢复。Objective To investigate the application effect of the blood flow restriction training(BFRT)combined with neuromuscular electrical stimulation(NMES)on the rehabilitation of the extensor lag of the quadriceps after knee arthroscopy.Methods A total of 60 patients who underwent knee arthroscopic surgery in Lushan Rehabilitation and Recuperation Center from June 2021 to December 2022 were selected.They were randomly divided into control group and observation group,with 30 cases per group.Rehabilitation plus NMES was performed in all patients,and those in the observation group were additionally given BFRT.Changes in the thigh circumference,range of the motion of the lagging joint,isokinetic strength of the quadriceps,and rehabilitation effects were compared between the two groups.Results At 4 and 6 weeks after treatment,the Hospital for Special Surgery Knee-Rating Scale(HSS)scores were significantly higher in the observation group than those of control group(4 weeks,[78.90±3.17]points VS[69.63±4.69]points;6 weeks,[89.20±5.12]points VS[79.93±3.63]points;P<0.05).At 4 and 6 weeks after treatment,significantly lower range of motion of the lagging joint and higher peak moment of isokinetic muscle of the quadriceps were detected in the observation group than those of control group(both P<0.01).At 6 weeks after treatment,the thigh circumference of the observation group was significantly improved compared with that of the control group in the same period(P<0.01).Conclusion BFRT combined with NMES has a significant effect on the rehabilitation of patients with the extensor lag of the quadriceps after knee arthroscopy.
