燔针劫刺针法联合常规针刺治疗瘀血痹阻型下肢静脉曲张临床观察

摘　　要：目的观察燔针劫刺针法联合常规针刺治疗下肢静脉曲张临床效果。方法以2022年1月至12月就诊于淮北市中医医院的60例下肢静脉曲张患者为研究对象,随机数字表分为对照组(常规针刺组,n=30)和观察组(燔针劫刺针法联合常规针刺组,n=30)。观察治疗前后两组患者患处酸胀疼痛、皮色暗红或紫暗、皮肤发凉干燥等血痹阻证相关中医证候积分、临床表现严重程度评分(VCSS)及静脉功能损害程度评分(VDS)、患者血浆黏度、纤维蛋白原和红细胞沉降率等血液流变学指标、血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-17(IL-17)等炎性因子的变化情况,观察两组总有效率,分析两组患者的临床治疗效果。结果治疗后,两组患者酸胀疼痛、皮色暗红或紫暗、皮肤发凉干燥等血痹阻证相关中医证候积分均明显降低(P<0.05),且观察组的各项中医证候积分明显低于对照组(P<0.05)。治疗后,两组患者VCSS、VDS评分均明显降低(P<0.05),且观察组患者VCSS、VDS评分明显低于对照组(P<0.05)。治疗后,两组患者血浆黏度、纤维蛋白原和红细胞沉降率均明显降低(P<0.05),且观察组血液流变学指标均低于对照组(P<0.05)。治疗后,两组患者血清CRP、TNF-α、IL-6、IL-17均降低,且观察组血清炎性因子均低于对照组(P<0.05)。观察组总有效率(93.33%)显著高于对照组(86.67%),差异有统计学意义(P<0.05)。结论燔针劫刺针法联合常规针刺治疗下肢静脉曲张疗效确切。Objective To observe the clinical effect of Fanzhen Jieci needling combined with conventional acupuncture on varicose veins of lower limbs with the type of blood stasis obstruction.Methods A total of 60 patients with varicose veins of lower limbs who were treated in Huaibei Hospital of Traditional Chinese Medicine from January 2022 to December 2022 were selected as the research objects.They were randomly divided into control group(conventional acupuncture,n=30)and observation group(Fanzhen Jieci needling combined with conventional acupuncture,n=30).Traditional Chinese medicine(TCM)syndrome scores of swelling and pain,dark red or purple skin color,cold and dry skin and other manifestations of blood stasis obstruction,the Venous Clinical Severity Score(VCSS),the Venous Disability Score(VDS),hemorheological indicators like the plasma viscosity(PV),fibrinogen,and erythrocyte sedimentation rate(ESR),and inflammatory factors like the C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-17(IL-17)were compared between groups.The total efficacy was compared between groups to analyze the therapeutic outcome.Results After treatment,the TCM syndrome scores of swelling and pain,dark red or purple skin color,cold and dry skin and other manifestations of blood stasis obstruction were significantly reduced in the both groups(P<0.05),which were significantly lower in the observation group than those of control group(P<0.05).After treatment,the VCSS and VDS scores in the both groups were significantly reduced(P<0.05),which were significantly lower in the observation group than those of control group(P<0.05).After treatment,PV,fibrinogen and ESR were significantly reduced in the both groups(P<0.05),which were significantly lower in the observation group than those of control group(P<0.05).After treatment,serum CRP,TNF-α,IL-6 and IL-17 were significantly reduced in the both groups,which were significantly lower in the observation group than those of control group(P<0.05).The overall effective
