腹腔镜下完全腹膜外疝修补术与开放填充式无张力疝修补术的临床疗效比较

摘　　要：目的 探讨在腹股沟疝治疗中应用腹腔镜下完全腹膜外疝修补术(TEP)与开放填充式无张力疝修补术(OP)的效果差异。方法 选择2017年7月—2020年12月上饶市妇幼保健院收治的80例腹股沟疝患者为研究对象,按治疗方式不同分组,各40例。对照组采用OP术治疗,观察组实施TEP术治疗,比较2组手术效果。结果 观察组手术用时长于对照组,住院费用高于对照组,而术中出血量、住院时间、下床活动时间和VAS评分均低于对照组,术后3 d、7 d C-反应蛋白(CRP)、血清白细胞介素-8(IL-8)和肿瘤坏死因子-α(TNF-α)水平低于对照组而胃动素(MTL)和胃泌素(GAS)水平高于对照组(P<0.05);2组并发症比较差异无统计学意义(P>0.05)。结论 TEP治疗腹股沟疝相较于OP其出血量更少,恢复时间更短且对炎症反应和胃肠功能影响较小,但手术时间和费用较高,临床需根据患者个体情况酌情选择。Objective To investigate the effect of laparoscopic total extraperitoneal hernia repair(TEP)and open filling tension-free hernia repair(OP)in the treatment of inguinal hernia.Methods A total of 80 patients with inguinal hernia admitted to our hospital from July 2017 to December 2020 were selected as the research objects,and they were divided into 40 cases in each group according to the treatment style.OP was used in the control group,TEP was used in the observation group,and the surgical effects of the two groups were compared.Results The operation time of the observation group was longer than that of the control group,the hospitalization cost was higher than that of the control group,and the intraoperative blood loss,hospitalization time,ambulation time and VAS score were all lower than those of the control group.The level ofαwas lower than that of the control group,while the levels of MTL and GAS were higher than those of the control group(P<0.05).There was no difference in complications between the two groups(P>0.05).Conclusion Compared with OP,TEP treatment of inguinal hernia has lower blood loss,shorter recovery time and less impact on inflammatory response and gastrointestinal function,but the operation time and cost are higher.
