β-HCG联合疼痛数字评分法评估宫外孕患者病情危重程度

摘　　要：目的:研究β-人绒毛膜促性腺激素(β-HCG)联合疼痛数字评分法(NRS)评估宫外孕患者病情危重程度。方法:选取2018年5月至21年5月我院收治的86例宫外孕患者,根据改良式加拿大急诊预检标尺(CTAS)预检分诊系统对患者病情分级;同期选50例体检的正常妊娠孕妇作为对照。检测受检者血清β-HCG,并采用NRS评估其疼痛程度,比较各组血清β-HCG水平、NRS评分,采用Pearson法分析血清β-HCG、NRS评分与宫外孕病情关系,并通过ROC曲线分析其对宫外孕诊断效能。结果:改良式CTAS评估结果显示,86例宫外孕患者病情危重程度分级为:Ⅰ级27例、Ⅱ级24例、Ⅲ级16例、Ⅳ级19例。宫外孕组血清β-HCG水平低于正常组,NRS评分高于正常组(P<0.05)。血清β-HCG水平、NRS评分在Ⅰ~Ⅳ级患者中存在差异,具有统计学意义(P<0.05)。Pearson分析显示,宫外孕病情危重程度与血清β-HCG水平呈负相关,与NRS评分呈正相关(P<0.05)。ROC曲线分析,β-HCG的灵敏度、特异度分别为82.6%、74.0%,NRS评分的灵敏度、特异度分别为84.9%、70.0%,两者联合的的灵敏度、特异度分别为94.2%、84.0%。结论:β-HCG与NRS与宫外孕危重程度密切相关,两者联合可评估患者病情危重程度,使患者得到及时救治,可作为临床评估宫外孕方式。Objective:Studyβ-Human chorionic gonadotropin(β-HCG)combined with digital pain score(NRS)to evaluate the severity of ectopic pregnancy.Methods:86 patients with ectopic pregnancy admitted to our hospital from May 2018 to May 21 were selected and classified according to the modified Canadian emergency pre examination scale(CTAs)pre examination and triage system;At the same time,50 normal pregnant women with physical examination were selected as controls.Detect the serum of the subjectβ-HCG,and NRS was used to evaluate the pain degree,and the serum of each group was comparedβ-HCG level and NRS score were analyzed by Pearson methodβ-The relationship between hCG and NRS scores and the condition of ectopic pregnancy was analyzed by ROC curve.Results:the modified CTAs evaluation results showed that 86 patients with ectopic pregnancy were classified into gradeⅠ(27 cases),gradeⅡ(24 cases),gradeⅢ(16 cases)and gradeⅣ(19 cases).Serum of ectopic pregnancy groupβ-The level of hCG was lower than that of the normal group and the NRS score was higher than that of the normal group(P<0.05).serumβ-There were significant differences in hCG level and NRS score among patients of gradeⅠ-Ⅳ(P<0.05).Pearson analysis showed that the severity of ectopic pregnancy was related to serumβ-HCG level was negatively correlated with NRS score(P<0.05).ROC curve analysis,β-The sensitivity and specificity of hCG were 82.6%and 74.0%respectively.The sensitivity and specificity of NRs score were 84.9%and 70.0%respectively.The sensitivity and specificity of the combination of the two were 94.2%and 84.0%respectively.Conclusion:β-HCG and NRs are closely related to the severity of ectopic pregnancy.The combination of hCG and NRS can assess the severity of the patient's condition,so that the patient can be treated in a timely manner,and can be used as a method for clinical evaluation of ectopic pregnancy.
