医院-社区-家庭联动管理在骨质疏松性椎体压缩骨折患者在唑来膦酸治疗期间的效果

摘　　要：目的:探究医院-社区-家庭联动管理应用于唑来膦酸治疗骨质疏松性椎体压缩骨折患者的效果。方法:选取2021年1月—2022年3月于我院骨科就诊的骨质疏松性椎体压缩性骨折患者87例,随机分为对照组(43例)和观察组(44例)。两组均予以唑来膦酸治疗,在此基础上,对照组予以常规管理,观察组予以医院-社区-家庭联动管理。比较两组用药依从性、出院时和出院半年后的自我管理能力评分、心理状态、疼痛情况以及出院1年内的骨折复发率。结果:出院半年后,观察组用药依从性、自我管理技能、自我概念、自我责任感及健康知识水平均高于对照组,SAS评分、SDS评分及VAS评分均低于对照组(P<0.05)。出院1年内,观察组骨折复发率低于对照组(P<0.05)。结论:医院-社区-家庭联动管理明显提升唑来膦酸治疗骨质疏松性椎体压缩骨折患者的用药依从性和自我管理能力,改善患者的心理状态,稳固唑来膦酸的治疗效果,降低患者疼痛程度,减少骨折复发情况。Objective:To explore the effect of hospital-community-family linkage management on patients with osteoporotic vertebral compression fractures treated with zoledronic acid.Methods:A total of 87 patients with osteoporotic vertebral compression fractures who were treated in our hospital from January 2021 to March 2022 were randomly divided into control group(43 patients)and observation group(44 patients).Both groups were treated with zoledronic acid.On this basis,the control group was given routine management,and the observation group was given hospital-community-family linkage management.The medication compliance,self-management ability score at discharge and half a year after discharge,psychological state,pain and fracture recurrence rate within 1 year after discharge were compared between the two groups.Results:Six months after discharge,the medication compliance,self-management skills,self-concept,self-responsibility and health knowledge level of the observation group were higher than those of the control group,and the SAS score,SDS score and VAS score were lower than those of the control group(P<0.05).Within 1 year after discharge,the recurrence rate of fracture in the observation group was lower than that in the control group(P<0.05).Conclusion:Hospital-community-family linkage management can significantly improve the medication compliance and self-management ability of patients with osteoporotic vertebral compression fractures treated with zoledronic acid,improve the psychological state of patients,stabilize the therapeutic effect of zoledronic acid,reduce the pain degree of patients,and reduce the recurrence of fractures.
