摘 要:目的探讨胎儿生长受限(FGR)的孕妇胎盘菌群特征。方法招募2021年5月至2022年5月在长沙市妇幼保健院行剖宫产孕妇的胎盘为研究样本;FGR组胎盘取自临床定义为新生儿出生体重小于同胎龄第10百分位数的孕妇20例,健康对照组胎盘来自孕周≥37周分娩且无其他合并症的孕妇20例。使用16S rRNA测序对胎盘微生物组进行调查,并确定与FGR相关的差异分类群特征。结果FGR组胎盘微生物的香农指数高于健康对照组、辛普森指数低于健康对照组(U值分别为4.381和5.012,P<0.05)。两组胎盘微生物的优势门为变形菌门、拟杆菌门、厚壁菌门和放线菌门;与健康对照组相比,FGR组变形菌门、拟杆菌门、厚壁菌门和放线菌门的比例均有降低(U值分别为3.891、4.264、3.702、5.476,P<0.05)。在门水平方面,与健康对照组相比,FGR组变形菌门、厚壁菌门和芽单胞菌门的比例均有降低(U值分别为3.642、3.180、2.826,P<0.05),而未分类细菌门的比例有所增高(U=4.513,P<0.01)。在科水平方面,与健康对照组相比,FGR组产碱菌科和毛螺菌科的比例均较低,而未分类k_norank菌科的比例较高(U值分别为4.746、3.281、4.895,P<0.05)。在属水平方面,两组有13个属不同;与健康对照组相比,FGR组新鞘氨醇菌属、无色杆菌属、不动杆菌属、副球菌属、假诺卡氏菌属、木洞菌属、硝化菌属、佩罗单胞菌属、产碱菌科未分类属和毛螺菌科未分类属的比例均有降低(U=1.935~5.174,P<0.05),而地发菌属、普雷沃氏菌科_NK3B31属和未分类细菌属的比例均有增高(U值分别为3.605、3.410、4.895,P<0.05)。结论FGR中存在明显的胎盘微生物群特征。Objective To explore characteristics of placental microflora in pregnant women with fetal growth restriction(FGR).Methods The placenta specimen from women who underwent cesarean section and gave a birth in Changsha Municipal Maternal and Child Health Hospital from May 2021 and May 2022 were recruited for study.20 placenta specimen were derived from those pregnant women with FGR(FGR group)whose infants'birth weight was clinically defined as the 1oth percentile small for gestational age,and other 20 placenta specimen in the control group were derived from the healthy pregnant women who delivered a gestational age-appropriate fetus at≥37 weeks of gestation without any other comorbidities.The placental microbiomes were investigated using 16S rRNA sequencing,and differential taxonomic signatures associated with FGR were identified.Results Shannon index of placental microorganisms in the FGR group was higher than that in the healthy control group,while Simpson index was lower than that in the healthy control group(U=4.381 and 5.012 respectively,both P<0.05).The dominant phyla of bacteria in the placenta in the two groups were Proteobacteria,Bacteroidetes,Firmicutes and Actinobacteria.The proportions of Proteobacteria,Bacteroidetes,Firmicutes and Actinobacteria in the FGR group decreased,as compared with the control group(U=3.891,4.264,3.702 and 5.476 respectively,all P<0.05).At phylum level,the proportions of Proteobacteria,Firmicutes and Gemmatinmonadetes in the FGR group decreased as compared with the control group(U=3.642,3.180 and 2.826 respectively,all P<0.05),while the proportion of unclassified bacteria phylum increased(U=4.513,P<0.01).At family level,the proportions of Alcaligenaceae and Lachnospiraceae were both lower,while that of Bacteria-unclassified was higher in the FGR group compared with the healthy control group(U=4.746,3.281 and 4.895 respectively,all P<0.05).At genus level,there were 13 different genera between the two groups.Compared with the healthy control group,the proportions of Novosphingobiu
