“双十字”型水胶体敷料联合新型鼻塞在早产儿经鼻持续气道正压通气中的应用效果研究

摘　　要：目的探讨“双十字”型水胶体敷料联合新型鼻塞在早产儿经鼻持续气道正压通气(NCPAP)中的应用效果。方法纳入肇庆市第一人民医院新生儿科2017年1月至2019年12月收治的50例应用NCPAP的早产儿(胎龄<37周,出生体质量<2500 g)为对照组,2020年1月至2022年12月收治的50例的应用NCPAP的早产儿为观察组。对照组采用“双十字”型水胶体敷料联合传统鼻塞的固定方法,观察组采用“双十字”型水胶体敷料联合新型鼻塞的改良固定方法。比较两组患儿干预后的血气指标、鼻塞脱落次数、经皮SPO_(2)、无创辅助通气时间以及并发症发生情况。结果治疗后,两组患儿PaCO_(2)、PaO_(2)及pH指标均有所改善,且观察组改善情况优于对照组;观察组在治疗期间鼻塞脱落次数、日平均SPO_(2)波动次数及NCPAP使用时间低于对照组;观察组并发症总发生率低于对照组(均P<0.05)。结论“双十字”型水胶体敷料联合新型鼻塞在早产儿NCPAP的临床应用中,可有效提高通气效果,减少鼻塞脱落的发生,有助于提高氧合水平,保持机体酸碱平衡,降低患儿的不利应激反应及并发症的发生,有效的促进患儿早日康复。Objective To explore the application effect of"double-cross"type hydrocolloid dressing combined with novel nasal plug transnasal continuous positive airway pressure ventilation(NCPAP)in preterm infants.Methods 50 cases of preterm infants(gestational age<37 weeks,birth weight<2500 g)admitted to the Neonatology Department of the First People's Hospital of Zhaoqing from January 2017 to December 2019 with NCPAP were included as the control group,and 50 cases of preterm infants admitted from January 2020 to December 2022 with NCPAP were included as the observation group.In the control group,the fixation method of“double-cross”hydrocolloid dressing combined with traditional nasal plugs was used,and in the observation group,the improved fixation method of“double-cross”hydrocolloid dressing combined with new nasal plugs was used.The blood gas index,the number of nasal plugs dislodged,the transcutaneous SPO_(2),the time of non-invasive assisted ventilation and the occurrence of complications were compared between the two groups after the intervention.Results After treatment,the PaCO_(2),PaO_(2)and pH indexes of the two groups improved,and the improvement of the observation group was better than that of the control group;the number of nasal plugs dislodged,the average number of SPO_(2)fluctuations per day,and the time of NCPAP use in the observation group were lower than that of the control group during the period of treatment;and the total incidence rate of complications was lower than that of the control group in the observation group(all P<0.05).Conclusions"Double cross"type hydrocolloid dressing combined with a new type of nasal plug in the clinical application of NCPAP in preterm infants can effectively improve the effect of ventilation,reduce the incidence of nasal plug dislodgement,help to improve the level of oxygenation,to maintain the acid-base balance of the body and reduce the occurrence of unfavorable stress reaction and complications in the children,and effectively promote the early recovery of the ch
