摘 要:目的分析99m锝-二乙三胺五乙酸(^(99m)Tc-DTPA)肾动态延迟显像的临床应用价值。方法117例行常规^(99m)Tc-DTPA肾动态检查单侧肾脏显像剂填充较差,或Gates法测得单侧肾小球滤过率(GFR)<15 ml/min的患者,于常规肾动态检查结束后30 min对其加作延迟静态显像,观察分析患肾显像剂的填充情况及肾动态延迟显像检查结果与临床不符情况、影响因素。结果117例患者的肾动态延迟显像图像中,患肾显影改善63例(显影良好39例,占比33.3%,部分改善24例,占比20.5%),其中肾积水伴结石29例,泌尿系结石30例,输尿管狭窄2例,肾肿瘤1例,肾萎缩1例;患肾显影较前相仿、无改善54例,占比46.2%。肾动态延迟显像检查结果与临床不符36例。结论^(99m)Tc-DTPA肾动态延迟显像通过延长显像时间,一定程度可以补充常规肾动态检查的不足,建议必要时可将肾动态延迟显像作为常规肾动态检查的补充,为临床提供更高的参考价值。Objective To analyze the clinical value of Tc 99m diethylenetriamine pentaacetic acid(^(99m)Tc-DTPA renal dynamic delayed imaging).Methods Delayed static imaging was performed 30 min after the end of the routine renal dynamic examination in 117 patients who had poor unilateral renal contrast filling in the routine ^(99m)Tc-DTPA renal dynamic examination or had a unilateral glomerular filtration rate(GFR)of<15 ml/min as measured by the Gates method.The filling of renal imaging agents,the inconsistencies between the results of renal dynamic delayed imaging and the clinical results and the influencing factors were observed and analyzed.Results Among the 117 patients with delayed renal dynamic imaging images,63 cases had improved renal imaging(39 cases with good imaging,accounting for 33.3%;24 cases with partial improvement,accounting for 20.5%),including 29 cases of hydronephrosis with calculi,30 cases of urinary calculi,2 cases of ureteral stenosis,1 case of renal tumor,and 1 case of renal atrophy.There were 54 cases(46.2%)with similar and no improvement in renal imaging.The results of renal dynamic delayed imaging were not consistent with clinical results in 36 cases.Conclusion ^(99m)Tc-DTPA renal dynamic delay imaging can supplement the deficiency of renal dynamic examination to a certain extent by extending the imaging time.It is suggested that renal dynamic delay imaging can be used as a supplement to routine renal dynamic examination when necessary,thus providing higher reference value for clinical practice.
