等速肌力锻炼系统对膝关节前交叉韧带重建术后功能恢复的影响

摘　　要：目的研究等速肌力锻炼系统对膝关节前交叉韧带重建术后功能恢复的影响。方法选取2020年1月至2022年1月西安交通大学附属红会医院康复医院收治的82例膝关节前交叉韧带重建术患者为研究对象,按随机数表法分为观察组(应用等速肌力锻炼系统)和对照组(应用常规康复训练方法)各41例,均训练1个月,比较两组患者术后1 d、1个月、3个月、6个月的疼痛视觉模拟量表(VAS)评分和膝关节功能恢复情况,入院时、术后1 d、术后1周、术后1个月大腿周径差值以及入院时、术后1个月等速肌力测定。结果术后1个月、3个月、6个月,观察组患者的VAS评分分别为(5.02±0.23)分、(3.25±0.63)分、(2.47±0.22)分,明显低于对照组的(6.37±0.31)分、(4.34±0.55)分、(3.89±0.17)分,差异均有统计学意义(P<0.05);术后1 d,两组患者的膝关节功能恢复评分比较差异无统计学意义(P>0.05),但术后1个月、3个月、6个月,观察组患者的膝关节功能恢复评分分别为(66.41±0.22)分、(72.39±0.47)分、(80.36±0.18)分,明显高于对照组的(61.67±0.19)分、(68.42±0.36)分、(75.23±0.21)分,差异均有统计学意义(P<0.05);入院时及术后1 d,两组患者的大腿周径差值比较差异均无统计学意义(P>0.05),但术后1周及术后1个月,观察组患者的大腿周径差值分别为(-0.35±0.22)cm、(0.12±0.03)cm,明显高于对照组的(-0.51±0.15)cm、(-0.41±0.34)cm,差异均有统计学意义(P<0.05);术前,两组患者的屈伸肌PT、TW、MRTW值比较差异均无统计学意义(P>0.05);术后1个月,观察组患者的屈肌和伸肌的PT、TW、MRTW值明显高于对照组,差异均有统计学意义(P<0.05)。结论等速肌力锻炼系统能够减轻膝关节前交叉韧带重建术后患者疼痛,加速膝关节功能恢复,改善下肢肿胀情况。Objective To study the effects of isokinetic muscle strength exercise system on functional recovery after knee anterior cruciate ligament reconstruction.Methods Eightytwo patients with knee anterior cruciate ligament reconstruction in Rehabilitation Hospital of Honghui Hospital Affiliated to Xi'an Jiaotong University from January 2020 to January 2022 were enrolled and divided into an observation group(isokinetic muscle strength exercise system)and a control group(conventional rehabilitation training method)by random number table method,with 41 cases in each group.Both groups were trained for 1 month.Visual Analogue Scale(VAS)score and knee function recovery at 1 day,1 month,3 months,and 6 months after surgery,thigh circumference difference at admission and at 1 day,1 week and 1 month after surgery,and isokinetic muscle strength measurement at admission and at 1 month after surgery were compared between the two groups.Results VAS scores in the observation group at 1 month,3 months,and 6 months after surgery were(5.02±0.23)points,(3.25±0.63)points,and(2.47±0.22)points,which were significantly lower than(6.37±0.31)points,(4.34±0.55)points,and(3.89±0.17)points in the control group(P<0.05).At 1 day after surgery,there was no statistically significant difference in knee function recovery score between the two groups(P>0.05).The knee function recovery scores in the observation group were(66.41±0.22)points,(72.39±0.47)points,and(80.36±0.18)points at 1 month,3 months,and 6 months after surgery,significantly higher than(61.67±0.19)points,(68.42±0.36)points,and(75.23±0.21)points in the control group(P<0.05).At admission and at 1 day after surgery,there was no statistically significant difference in the difference of thigh circumference between the two groups(P>0.05).At 1 week and 1 month after surgery,the difference values of thigh circumference were(-0.35±0.22)cm and(0.12±0.03)cm in the observation group,significantly higher than(-0.51±0.15)cm and(-0.41±0.34)cm in the control group(P<0.05).Before treatment,
