人工股骨头置换术与全髋关节置换术治疗老年移位股骨颈骨折的有效性评价

摘　　要：目的对比人工股骨头置换术、全髋关节置换术两种术法对老年移位股骨颈骨折的治疗有效性及安全性。方法回顾性分析2020年6月—2022年6月期间在江苏省淮安市洪泽区人民医院骨科治疗移位股骨颈骨折的70例老年患者的临床资料,按照手术方法,将20例采取全髋关节置换术的患者设为研究组,将50例采取人工股骨头置换术的患者设为对照组。对比两组手术效果。结果研究组术中失血量比对照组多,手术耗时、下床时间、住院时间比对照组长,差异有统计学意义(P<0.05)。治疗后,研究组髋关节功能评分、术后6个月优良率比对照组高,髋部疼痛评分比对照组低,差异有统计学意义(P<0.05)。研究组并发症发生率(20.00%)比对照组高(2.00%),差异有统计学意义(χ^(2)=4.529,P<0.05)。结论对于老年移位股骨颈骨折,人工股骨头置换术近期疗效好,全髋关节置换术远期疗效好,因此应为患者优选全髋关节置换术。Objective To compare the effectiveness and safety of artificial hemiarthroplasty and total hip arthroplasty in the treatment of displaced femoral neck fractures in the elderly.Methods Retrospectively analysed of the clinical data of 70 elderly patients who underwent treatment of displaced femoral neck fractures in the orthopedics department of Hongze District People's Hospital in Huai'an City,Jiangsu Province from June 2020 to June 2022.According to surgical methods,20 patients who underwent total hip replacement surgery were assigned to the study group,and 50 patients who underwent artificial femoral head replacement surgery were assigned to the control group.Compared the surgical outcomes between the two groups.Results The study group had more intraoperative blood loss than the control group,and the surgical time,bedtime,and hospital stay were significantly longer than those of the control group,the difference was statistically significant(P<0.05).After treatment,the hip joint function score and the excellent and good rate 3 months after surgery in the study group were higher than those in the control group,hip pain score was lower than the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the study group(20.00%)was higher than that in the control group(2.00%),the difference was statistically significant(χ^(2)=4.529,P<0.05).Conclusion For elderly displaced femoral neck fractures,artificial femoral head replacement has good short-term effects,and total hip replacement has good long-term effects.Therefore,total hip replacement should be the first choice for patients.
