加味八珍颗粒联合营养支持对结直肠癌术后患者的临床疗效

摘　　要：目的探讨加味八珍颗粒联合营养支持对结直肠癌术后患者的临床疗效。方法120例患者随机分为对照组和观察组,每组60例。2组均给予营养支持,同时对照组采用加味八珍颗粒模拟颗粒,观察组采用加味八珍颗粒,疗程4周。检测PG-SGA评分、CONUT评分、PNI、血清蛋白(ALB、PA、TP)、上臂肌围、BMI、T细胞(CD3^(+)、CD4^(+)、CD8^(+)、Treg、Th17、CD4^(+)/CD8^(+)、Th17/Treg)、NK细胞、免疫球蛋白(IgG、IgM、IgA)、免疫微环境指标(PD-1、TIM-3、CTLA-4)、免疫炎症指标(TNF-α、IL-6、IL-10、IFN-γ、NLR、LMR)变化。结果治疗后,2组PG-SGA评分、CONUT评分、CD8+、Th17、Th17/Treg、免疫微环境指标、TNF-α、IL-6、NLR降低(P<0.05),PNI、血清蛋白、免疫球蛋白、CD3^(+)、CD4^(+)、Treg、CD4^(+)/CD8^(+)、NK细胞、IL-10、IFN-γ、LMR升高(P<0.05),以观察组更明显(P<0.05);观察组上臂肌围增加(P<0.05),也大于对照组(P<0.05)。结论加味八珍颗粒联合营养支持可改善结直肠癌术后患者营养状态、免疫功能,调节免疫微环境、免疫炎症因子,重塑免疫微环境稳态,促进康复、预后。AIM To explore the clinical effects of Supplemented Bazhen Granules combined with nutritional support on patients after colorectal cancer surgery.METHODS One hundred and twenty patients were randomly assigned into control group(60 cases)for 4-week intervention of both simulated granules of Supplemented Bazhen Granules and nutritional support,and observation group(60 cases)for 4-week intervention of both Supplemented Bazhen Granules and nutritional support.The changes in PG-SGA score,CONUT score,PNI,serum proteins(ALB,PA,TP),upper arm muscle circumference,BMI,T cells(CD3^(+),CD4^(+),CD8^(+),Treg,Th17,CD4^(+)/CD8^(+),Th17/Treg),NK cells,immunoglobulins(IgG,IgM,IgA),immune microenvironment indices(PD-1,TIM-3,CTLA-4)and immune-inflammatory indices(TNF-α,IL-6,IL-10,IFN-γ,NLR,LMR)were detected.RESULTS After the treatment,the two groups displayed decreased PG-SGA score,CONUT score,CD8^(+),Th17,Th17/Treg,immune microenvironment indices,TNF-α,IL-6,NLR(P<0.05),and increased PNI,serum proteins,immunoglobulins,CD3^(+),CD4^(+),Treg,CD4^(+)/CD8^(+),NK cells,IL-10,IFN-γ,LMR(P<0.05),especially for the observation group(P<0.05);the observation group exhibited elevated upper arm muscle circumference(P<0.05),which was also bigger than that in the control group(P<0.05).CONCLUSION For the patients after colorectal cancer surgery,Supplemented Bazhen Granules combined with nutritional support can improve nutritional status and immune functions,regulate immune microenvironment and immune inflammatory factors,remodel immune microenvironment homeostasis,and facilitate rehabilitation and prognosis.
