儿童照顾者居家用药安全评估量表的编制及信度效度检验

摘　　要：目的:编制儿童照顾者居家用药安全评估量表,并对量表进行信度、效度检验,以期为制定儿童照顾者居家用药安全提升方案提供指导。方法:以健康信念模式为理论框架,通过文献回顾、质性访谈构建量表条目池,经专家函询和预调查,形成预试量表。于2022年3月—4月采用便利抽样法选取250名儿童照顾者对预试量表进行项目分析后形成正式量表;于2022年5月—7月选取402名儿童照顾者进行正式调查,进一步分析量表的信度和效度。结果:儿童照顾者居家用药安全评估量表包含感知威胁、感知益处、感知障碍、自我效能、安全用药行为、潜在风险用药行为和不安全用药行为7个维度,共34个条目。探索性因子分析提取出7个公因子,累计方差贡献率为69.066%。验证性因子分析结果显示,正式量表模型适配度指标均在可接受范围内,模型拟合良好,量表因子结构稳定。量表平均内容效度指数为0.940,条目内容效度指数为0.838~1.000,量表的Cronbach’sα系数为0.848,折半信度为0.776,重测信度为0.880。结论:本研究制定的儿童照顾者居家用药安全评估量表具有良好的信度、效度,可作为评估儿童照顾者居家用药安全水平的科学工具,为制定儿童照顾者居家用药安全提升方案提供指导。Objective:To develop and evaluate the reliability and validity of Home Medication Safety Assessment Scale of Children's Caregivers.Methods:Based on the health belief model,the initial scale was firstly developed through literature review and qualitative interview of children's caregivers.The final scale was formed by expert consultation and pre-investigation.From March to April 2022,a total of 250 cases of children caregivers were selected by convenience sampling for item analysis and factor analysis.From May to July 2022,totally 402 children caregivers were selected for questionnaire survey to further analyze the reliability and validity of the scale.Results:There were 34 items in the formal scale,with 7 dimensions including perceived threat,perceived benefits,perceived obstacles,self-efficacy,safe medication behavior,potentially risky medication behavior and unsafe medication behavior.Seven common factors were extracted from exploratory factor analysis,and the cumulative variance contribution rate was 69.066%.Confirmatory factor analysis showed that the model fit was good and the scale factor structure was stable.The content validity index of each item was 0.838-1.000;the content validity index of the average scale level was 0.940;the Cronbach'sαcoefficient of the scale was 0.848;the split-half reliability was 0.776;and the test-retest reliability was 0.880.Conclusion:The home medication safety assessment scale of children's caregivers developed in this study has a good reliability and validity,which can be used as a scientific tool to evaluate the home medication safety of children caregivers,and provide guidance for the development of home medication safety improvement programs for children caregivers.
