H-FABP在早期心肌梗死中的诊断价值的临床观察

摘　　要：目的探究早期心肌梗死诊断中,心脏型脂肪酸结合蛋白(H-FABP)水平作为指标的临床应用价值。方法选取心肌梗死患者70例为心肌梗死组,同时选取健康的63名体检者为健康对照组。采集静脉血进行H-FABP、肌红蛋白(MYO)、肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白Ⅰ(cTnI)检测。结果与健康对照组比较,心肌梗死组血清MYO、CKMB、cTnI表达水平升高,差异有统计学意义(P<0.001);与健康对照组比较,心肌梗死组血清H-FABP表达显著上调(P<0.001),且其水平与MYO,CK-MB,cTnI呈现正相关(P<0.05);受试者工作特征曲线(ROC)结果显示,H-FABP的曲线下面积为0.904(95%CI:0.844-0.964),灵敏度、特异度较高。结论H-FABP水平对早期心肌梗死的诊断具有重要价值,能够为早期临床诊断治疗提供值得推广的依据。Objective To study the application value of heart-type fatty acid binding protein(H-FABP)in the diagnosis of early myocardial infarction.Methods 70 patients with myocardial infarction were selected as the myocardial infarction group,and as the same time,63 healthy examiners were selected as the healthy control group.The venous blood was collected to detect the levels of H-FABP,myoglobin(MYO),creatine kinase-MB isoenzyme(CK-MB)and cardiac troponin I(cTnI).Results Compared with the healthy control group,the expression levels of serum MYO,CK-MB and cTnI in the myocardial infarction group increased,and there was statistically significant difference(P<0.001).Compared with the healthy control group,the expression of H-FABP in the myocardial infarction group increased significantly(P<0.001),and its level was positively correlated with MYO,CK-MB and cTnI(P<0.05).The result of receiver operating characteristic curve(ROC)showed that the area under the curve of H-FABP was 0.904(95%CI:0.844-0.964),which had higher sensitivity and specificity.Conclusion H-FABP level examination has high application value in the diagnosis of early myocardial infarction,and can provide valuable promotion basis for early clinical diagnosis and treatment of this disease.
