摘 要:目的分析芳香烃受体(AhR)、25-羟维生素D(25-OHD)和胞质分裂蛋白8(DOCK8)在小儿特应性皮炎(AD)中的表达及相关性。方法选取2019年8月—2020年8月本院收治的92例AD患儿作为AD组,80例系统性红斑狼疮(SLE)患儿作为SLE组,以同期在本院健康体检的90例健康儿童作为健康对照组。测定3组研究对象AhR、25-OHD和DOCK8的mRNA表达情况。采用Logistic回归分析影响AD发生的影响因素,采用Pearson相关性分析法分析AhR、25-OHD和DOCK8的相关性,受试者工作特征曲线(ROC曲线)分析影响因素对AD发生的预测价值。结果与健康对照组比较,SLE组及AD组AhR、DOCK8的mRNA表达升高,25-OHD的mRNA表达降低,差异有统计学意义(P<0.05);与SLE组比较,AD组AhR、DOCK8的mRNA表达升高,25-OHD的mRNA表达降低,差异有统计学意义(P<0.05)。随着疾病进展,AD患儿AhR、DOCK8的mRNA表达升高,25-OHD的mRNA表达降低。与轻度AD患儿比较,中度、重度AD患儿AhR、DOCK8的mRNA表达升高,25-OHD的mRNA表达降低(P<0.05)。与中度比较,重度AD患儿AhR、DOCK8的mRNA表达升高,25-OHD的mRNA表达降低(P<0.05)。以小儿AD作为因变量(否=1,是=0),以AhR、25-OHD、DOCK8作为自变量纳入方程,建立Logistic回归模型,结果显示,高AhR[优势比(OR)=13.283,95%可信区间(95%CI):4.371~40.365]和高DOCK8(OR=3402.792,95%CI:100.332~115406.258)为小儿AD发生的独立危险因素,高25-OHD(OR=0.679,95%CI:0.561~0.822)为小儿AD发生的保护因素(均P<0.05)。Pearson相关性分析结果显示,AhR与25-OHD之间呈负相关(r=-0.230,P=0.003);AhR与DOCK8之间呈正相关(r=0.410,P=0.001);25-OHD与DOCK8之间呈负相关(r=-0.455,P=0.001)。与AhR(AUC=0.897)、25-OHD(AUC=0.766)和DOCK8(AUC=0.918)单项预测比较,3项联合(AUC=0.975,当截断值为0.431时,其敏感度为98.24%,特异度为89.13%)预测小儿AD发生的价值较高(P<0.05)。结论AhR在小儿AD中表达升高,DOCK8、25-OHD在小儿AD中表达降低,且与患儿疾病严重程度相关,三者线性�Objective In order to analyze the correlated expression of aromatic hydrocarbon receptor(AhR),25-hydroxyvitamin D(25-OHD),and dedicator of cytokiniesis 8(DOCK8)in children with atopic dermatitis(AD).Methods A total of 92 children with AD admitted to our hospital from August 2019 to August 2020 were selected as the AD group,80 children with SLE as the SLE group,and 90 healthy children who underwent physical examination as the healthy control group.The mRNA expression of AhR,25-OHD,DOCK8 were measured.Logistic regression analysis was used to analyze the influence factors that affect the occurrence of AD.Pearson correlation analysis was used to analyze the correlation between AhR,25-OHD,and DOCK8.Receiver operator characteristic curve(ROC curve)was drawn to analyze the predictive value of influence factors on the occurrence of AD.Results Compared with the healthy control group,the mRNA expression of AhR and DOCK8 in the SLE group and the AD group increased,and the mRNA expression of 25-OHD decreased,and the difference was statistically significant(all P<0.05);compared with the SLE group,the mRNA expression of AhR and DOCK8 in the AD group increased,the mRNA expression of 25-OHD decreased,and the difference was statistically significant(all P<0.05).As the disease progressed,the expression of AhR and DOCK8 in AD children increases,and the mRNA expression of 25-OHD decreases.Compared with mild,the expression of AhR and DOCK8 in children with moderate and severe AD increased,and the mRNA expression of 25-OHD decreased(both P<0.05).Compared with moderate,the mRNA expression of AhR and DOCK8 was increased in children with severe AD,and the mRNA expression of 25-OHD was decreased(P<0.05).With children's AD as the dependent variable(no=1,yes=0),AhR,25-OHD,and DOCK8 were used as independent variables into the equation to establish a Logistic regression model.The results showed that high AhR(OR=13.283,95%CI:4.371~40.365)and high DOCK8(OR=3402.792,95%CI:100.332~115406.258)were independent risk factors for pediatric AD,while hi
