超声引导下腘窝坐骨神经阻滞在足部手术中的应用效果

摘　　要：目的:分析超声引导下腘窝坐骨神经阻滞在足部手术中的应用效果。方法:选取2021年1月—2022年10月贵州盘江煤电集团有限责任公司医院收治的接受足部手术患者50例为研究对象,根据随机数字表法分为参照组(25例)和研究组(25例)。参照组应用蛛网膜下腔阻滞麻醉,研究组应用超声引导下腘窝坐骨神经阻滞麻醉。比较两组患者麻醉操作时间、运动阻滞与感觉阻滞持续时间、心率与平均动脉后水平及不良反应发生情况。结果:两组患者麻醉操作时间比较,差异无统计学意义(P>0.05);研究组运动阻滞持续时间、感觉阻滞持续时间短于参照组,差异有统计学意义(P=0.001)。麻醉前及麻醉后各时间段,两组患者心率比较,差异无统计学意义(P>0.05);麻醉前,两组患者平均动脉压水平比较,差异无统计学意义(P>0.05),研究组麻醉后各时间段平均动脉压水平低于参照组,差异有统计学意义(P=0.001)。研究组不良反应发生率低于参照组,差异有统计学意义(P=0.001)。结论:足部手术患者采用超声引导下腘窝坐骨神经阻滞麻醉可以提升麻醉效果,延长患者麻醉效果持续时间,对平均动脉压水平影响较小,安全性较高。Objective:To analyze the analgesic effect of ultrasound-guided popliteal Fossa sciatic nerve block in foot surgery.Methods:50 patients admitted to Guizhou Panjiang Coal and Electricity Group Hospital to undergo foot surgery from January 2021 to October 2022 were selected as the study subjects,and were divided into the reference group(25 cases)and the study group(25 cases)according to the method of randomized numerical table.The reference group was anesthetized with subarachnoid block while and the study group with popliteal sciatic nerve block under ultrasound guidance.The following indexes such as anesthesia operation duration,duration of motor block and sensory block,heart rate and mean post-arterial level,and the occurrence of adverse reactions were compared between the two groups.Results:Comparing the anesthesia operation duration of the two groups of patients does not result into a statistically significant difference(P>0.05);the duration of motor block and sensory block in the study group was shorter than that in the reference group,and the difference was statistically significant(P=0.001).Before anesthesia and after anesthesia time periods,the heart rate of patients in the two groups was compared,and the difference was not statistically significant(P>0.05);before anesthesia,the average arterial pressure level of patients in the two groups was compared,and the difference was not statistically significant(P>0.05),and the average arterial pressure indexes of the research group in varied post-anesthetic periods was lower than those of the reference group,and the difference was statistically significant(P=0.001).The incidence of adverse reactions in the study group was lower than that in the reference group,and the difference was statistically significant(P=0.001).Conclusion:The application of ultrasound-guided popliteal sciatic nerve block in patients undergoing foot surgery is safe and it can enhance the anesthetic effect,prolong the duration of the patient's anesthetic effect,and have less impact on the aver
