DBT结合动态增强MRI对不典型乳腺癌的诊断价值分析

摘　　要：探讨乳腺数字影像断层摄影(Digital Bredst Tomosynthesis,DBT)结合动态增强MRI扫描对不典型乳腺癌的诊断价值,提高不典型乳腺癌的诊断率。选取江南大学附属医院近两年来经DBT及动态增强MRI扫描确诊以及同时经手术病理证实的60例乳腺癌患者为研究对象,分析DBT及动态增强MRI扫描的影像学征象对不典型乳腺癌的诊断价值。结果显示,60例不典型乳腺癌患者中35例有明显的不成形肿块影,少部分块影有分叶及毛刺样改变,15例有孤立丛状、小线虫状、泥沙或针尖状、小杆状、粉刺样不定型钙化,其中11例同时伴有腺体纠集、乳头凹陷及局部皮肤牵拉等间接征象。研究发现,DBT及动态增强MRI扫描对不典型乳腺癌的影像诊断具有重要意义,尤其是对于临床触诊阳性的患者来说尤其重要,以上两种乳腺影像学特殊检查更是必不可少的诊断方法。To explore the diagnostic value of digital bredst tomography(DBT)combined with dynamic enhanced MRI scan for atypical breast cancer,and improve the diagnostic rate of atypical breast cancer.In the past two years,60 breast cancer patients diagnosed by DBT and dynamic enhanced MRI scan and confirmed by surgical pathology were selected as the research subjects,and the diagnostic value of imaging signs of DBT and dynamic enhanced MRI scan for atypical breast cancer was analyzed.The results showed that 35 of the 60 patients with atypical breast cancer had obvious asympathetic mass opacities,a small number of lumps had lobulation and burr-like changes,15 cases had isolated plexiform,small nematodes,sediment or needle point,small rod-like and acne-like amorphous calcification,of which 11 cases were accompanied by indirect signs such as gland entanglement,nipple depression and local skin traction.DBT and dynamic enhanced MRI scans have been found to be of great significance for the imaging diagnosis of atypical breast cancer,especially for patients with positive clinical palpation,and the above two special breast imaging tests are indispensable diagnostic methods.
