4R危机管理模式在ECMO患者抗凝护理中的应用研究

摘　　要：探讨4R危机管理模式在ECMO患者抗凝护理中的应用效果。选取2022年1月—2023年5月柳州市工人医院使用ECMO支持治疗的112例患者作为研究对象,基于随机数表法将其分为常规组和实验组。常规组有52例患者,实验组有60例患者,常规组采用常规监护的方法,实验组运用4R危机管理理论建立护理模式进行护理。比较两组患者不同护理模式下抗凝相关并发症发生率以及患者SOFA及APACHEⅡ评分。结果显示,实验组抗凝相关并发症发生率为26.67%(16/60),常规组的抗凝相关并发症发生率为34.62%(18/52),实验组抗凝相关并发症发生率低于常规组(χ^(2)=5.879,P=0.015)。干预后,实验组患者的SOFA和APACHEⅡ评分显著降低(P<0.05)。研究发现,4R危机管理模式应用于ECMO患者抗凝护理过程中,可明显改善患者的症状与体征。Explore the application effect of the 4R crisis management model in anticoagulant care for ECMO patients.112 pa-tients who received ECMO support treatment at Liuzhou Workers’Hospital from January 2022 to May 2023 were selected as the research subjects,and were divided into a conventional group and an experimental group based on a random number table method.There are 52 patients in the conventional group and 60 patients in the experimental group.The conventional group adopts routine monitoring methods,and the experimental group uses the 4R crisis management theory to establish a nursing model for nursing.Compare the incidence of anticoagulation related complications and the SOFA and APACHE II scores of two groups of patients under different nursing modes.The results showed that the incidence of anticoagulation related complications in the experimental group was 26.67%(16/60),while the incidence of anticoagulation related complications in the conventional group was 34.62%(18/52).The incidence of anticoagulation related complications in the experimental group was lower than that in the conventional group(χ^(2)=5.879,P=0.015).After intervention,the SOFA and APACHE II scores of the experimental group patients were signifi-cantly reduced(P<0.05).Research has found that applying the 4R crisis management model to the anticoagulation nursing process of ECMO patients can significantly improve their symptoms and signs.
