摘  要:目的 探讨针刀干预对颈型颈椎病模型兔颈深部多裂肌血流变化的影响及对多裂肌细胞中Wnt信号蛋白-3α(Wnt3α)、β-连环蛋白(β-catenin)和肌球蛋白重链-I(MyHC1)表达的影响。方法 将24只6月龄雄性新西兰兔分为空白组、模型组、针刀组,采用低头位方法建立异常应力诱导下的颈型颈椎病兔模型,8周后通过X线摄片验证造模结果。在造模成功后第3天进行干预,以针刀四步法为指导,每周1次,共干预4周。模型组、针刀组采用相同固定方式固定。结束后采用多普勒超声法观测各组实验兔颈部肌肉血流供应状况变化;Western blot法和q-PCR法检测颈部深层多裂肌中Wnt3α、β-catenin、MyHC1蛋白和基因含量的变化。结果 多普勒超声显示:空白组家兔颈部肌肉结构完整肌束连续,肌束呈中低水平回声,肌束膜呈强回声。血流信号呈点状分布。模型组家兔颈部肌肉结构完整性被破坏,肌肉组织内肌丝回声增强,肌束膜和肌内结缔组织网络样回声断续。整体相较空白组回声信号偏高。血流信号较空白组丰富,由点状融合成片状。针刀组家兔颈部肌肉结构完整性较模型组有所改善,肌束膜连续性有所恢复。血流信号呈点状分布,但较空白组仍可见血流信号增多;Western blot法结果显示:与空白组相比,模型组颈多裂肌中Wnt3α、β-catenin、MyHC1蛋白表达降低(P<0.05);与模型组相比,针刀组中Wnt3α、β-catenin、MyHC1蛋白表达增加(P<0.05);q-PCR法结果显示:与空白组相比,模型组颈多裂肌中Wnt3α、β-catenin、MyHC1基因表达降低(P<0.05);与模型组相比,针刀组中Wnt3α、β-catenin、MyHC1基因表达增高(P<0.05)。结论 实验所选取的颈型颈椎病兔模型具有可行性;针刀干预可以减轻炎症,促进肌组织供血恢复;针刀治疗修复颈型颈椎病受损颈肌的可能机制是通过干预调整Wnt/β-catenin信号通路,从而达到促进颈部肌肉修复,改善颈椎力�Objective To investigate the effect of acupotomy intervention on the changes of blood flow in the deep multifidus muscle of the cervical spondylosis model rabbits and the expressions of Wnt3α,β-catenin and MyHC1 in multifidus muscle cells.s Methods Twenty-four 6-month-old male New Zealand rabbits were divided into blank group,model group and acupoto-my group according to random number table method,with 8 rabbits in each group.The success of the modeling was verified by X-ray after a week.The intervention was performed on the 3rd day after the successful modeling,guided by the four-step acu-potomy method under sterile conditions,once a week for a total of 4 weeks of intervention.The model group and acupotomy group were fixed in the same way.After the end of the study,Doppler ultrasound was used to observe the changes of blood supply in the neck muscles of the experimental rabbits in each group.Western Blot and qPCR methods were used to detect the changes of Wnt3α,β-catenin,MyHC1 protein and gene contents in the deep cervical multifidus muscle.s Results Doppler ultrasonography showed that the neck muscle structure of the rabbits in the blank group was intact and the muscle bundles were continuous.The blood flow signal was punctate.In the model group,the structural integrity of the neck muscles of the rabbits was destroyed,the echoes of the myofilaments in the muscle tissue were enhanced,and the echoes of the perimysium and the intramuscular connec-tive tissue network were intermittent.The overall echo signal was higher than that of the blank group,suggesting that there may be inflammation.The blood flow signal was more abundant than that in the blank group,which was fused from dots into sheets,sug-gesting that there may be proliferation of small blood vessels.Compared with those of the model group,the structural integrity of the neck muscles of the rabbits in the acupotomy group was improved,and the continuity of the perimysium was restored.The blood flow signal was distributed in dots,but compared with that of
