摘　　要：目的 探究半夏泻心汤分阶段综合治疗难治性幽门螺杆菌感染(RHPI)的效果。方法 选取于2021年1月-2022年10月收治入医院的122例RHPI患者作为研究对象,按照随机数字表法将其分成对照组61例和试验组61例。所有患者均采取三阶段综合治疗,第二关键阶段均采取四联抗菌治疗,对照组第一准备阶段和第三巩固阶段采取保护胃黏膜及促进胃动力治疗;试验组第一准备阶段和第三巩固阶段均采取半夏泻心汤治疗。比较组间患者临床症状情况、幽门螺杆菌(Helicobacter pylori, Hp)根除率及复感率。结果 治疗前,组间中医各项证候(嘈杂吞酸、纳呆少食、肠鸣、便溏不爽、胃脘疼痛、恶心呕吐、痞闷胀满、口苦口干、嗳气烧心)评分比较,差异无统计学意义(P>0.05);治疗后,试验组上述证候评分均明显低于对照组(P<0.05)。停药一个月后,试验组Hp根除率明显高于对照组(P<0.05)。停药一年后,试验组Hp复感率明显低于对照组(P<0.05)。结论 半夏泻心汤分阶段综合治疗RHPI,有助于缓解患者临床症状,有助于提高Hp根除率,降低Hp复感率。Objective To explore the effect of Banxia Xiexin Decoction in the treatment of RHPI.Methods 122 patients with Refractory helicobacter pylori infection(RHPI)admitted to our hospital from January 2021 to October 2022 were selected as the objects of study.They were divided into control group(n=61 cases)and experimental group(n=61 cases)according to ran-dom number table method.All patients received three-stage comprehensive treatment,the second critical stage was treated with four antibiotics,the control group was treated with gastric mucosa protection and gastric motility promotion at the first preparation stage and the third consolidation stage.The experimental group was treated with Banxia Xiexin Decoction at the first preparation stage and the third consolidation stage.The clinical symptoms,eradication rate of Helicobacter pylori(Hp)and relapse rate were compared between groups.Results Before treatment,there was no statistical significance in the scores of various TCM syn-droms(noisy and acid swallowing,lethargy and less food,bowel song,loose stool,epigastric pain,nausea and vomiting,full-ness of fullness,bitter mouth and dry mouth,belching and heartburn)between groups(P>0.05).After treatment,the above syndrome scores of experimental group were significantly lower than those of control group(P<0.05).One month after with-drawal,the eradication rate of Hp in experimental group was significantly higher than that in control group(P<0.05).After one year of drug withdrawal,the Hp relapse rate in experimental group was significantly lower than that in control group(P<0.05).Conclusion Banxia Xiexin Decoction in the treatment of RHPI can relieve the clinical symptoms,improve the eradication rate of Hp and reduce the rate of Hp relapse.
