加味芎归二陈汤联合五积散对痰湿型多囊卵巢综合征患者血流动力学、糖脂代谢影响研究

摘　　要：目的 分析加味芎归二陈汤联合五积散对痰湿型多囊卵巢综合征患者血流动力学、糖脂代谢的影响。方法 选取2021年1月—2022年6月,于河北省沧州中西医结合医院选取痰湿型多囊卵巢综合征患者82例,随机分为3组,五积散组27例,加味芎归二陈汤组27例,联合组28例。五积散组给予五积散治疗,加味芎归二陈汤组给予加味芎归二陈汤治疗,联合组给予加味芎归二陈汤联合五积散治疗。评估中医证候积分;酶联免疫吸附试验法对促卵泡激素(follicle-stimulating hormone,FSH)、孕酮(progesterone,P)、促黄体生成素(luteinizing hormone,LH)水平进行检测;用B超对3组患者卵泡数目、卵巢体积等情况进行记录;采用血流检测仪器检测收缩期峰值流速(peak systolic velocity,PSV)、血流灌注指数(perfusion index,PI)、血流阻力指数(blood flow resistance index,RI)水平;采用葡萄糖氧化酶法检测糖化血红蛋白(glycated hemoglobin,HbAlc)、空腹胰岛素(fasting insulin,FINS)水平;使用全自动生化分析仪检测总胆固醇(total cholesterol,TC)、甘油三酯(triglycerides,TG)水平,对比3组治疗效果。结果 治疗后,与五积散组和加味芎归二陈汤组相比,联合组中医证候积分及P、RI、HbAlc、FINS、TC、TG水平均降低(P<0.05);卵泡数目、卵巢体积均减小(P<0.05);FSH、LH、PSV、PI水平升高(P<0.05);联合组总有效率升高(P<0.05)。结论 经加味芎归二陈汤联合五积散治疗后,痰湿型多囊卵巢综合征患者中医证候积分降低,血液动力学以及糖脂代谢水平均显著改善,联合治疗效果显著。Objective To analyze the effects of Modified Xionggui Erchen Decoction(加味芎归二陈汤)combined with Wuji Powder(五积散)on hemodynamics and glucose and lipid metabolism in patients with phlegm-dampness type polycystic ovary syndrome.Methods From January 2021 to June 2022,82 patients with phlegm-dampness type polycystic ovarian syndrome in Cangzhou hospital of integrated TCM-WM·Hebei were selected and randomly divided into three groups:Wuji Powder group(27 cases),Modified Xionggui Erchen Decoction group(27 cases),and combination group(28 cases).Wuji Powder group was treated with Wuji Powder.The Modified Xionggui Erchen Decoction group was given Modified Xionggui Erchen Decoction.The combined group was treated with Modified Xionggui Erchen Decoction combined with Wuji Powder.Evaluation of TCM syndrome scores;The levels of follicle-stimulating hormone(FSH),progesterone(P)and luteinizing hormone(LH)were detected by enzyme-linked immunosorbent assay.The number of follicles and ovarian volume of the two groups were recorded by B-mode ultrasound.The peak systolic velocity(PSV),perfusion index(PI)and blood flow resistance index(RI)were detected by blood flow detection instrument.Glucose oxidase was used to detect the levels of Glycated hemoglobin(HbAlc)and fasting insulin(FINS).The levels of total cholesterol(TC)and triglycerides(TG)were detected by automatic biochemical analyzer,and the treatment effects of the three groups were compared.Results After treatment,compared with the Wuji Powder group and the Modified Xionggui Erchen Decoction group,the TCM syndrome scores,P,RI,HbAlc,FINS,TC and TG levels of the combined group were all decreased(P<0.05);the number of follicles,the ovarian volume decreased(P<0.05);the levels of FSH,LH,PSV and PI increased(P<0.05);The total effective rate in the combined group increased(P<0.05).Conclusion After combined treatment with Modified Xionggui Erchen Decoction and Wuji Powder,the TCM syndrome scores of patients with phlegm-dampness type polycystic ovary syndrome was reduced,
