藏医学中便秘和尿闭症的关联性和致病机理探析

摘　　要：藏医《四部医典》之《秘诀医典》中与便秘和尿闭症相关的疾病比较常见,但其分类不明确,诊断易混淆,因其致病机理不仅在泌尿系统和肠道发生病变,而且与五脏六腑、外伤、合成毒症等有着密切的关联。临床上常以便秘和尿闭的症状为依据进行治疗,而不是以两种疾病进行辩证、分析和治疗,从而影响临床疗效。以藏医古籍《四部医典》等经典为依据探讨和分析便秘和尿闭症的关联性和致病因素。在藏医学中便秘和尿闭症是两种不同的疾病,这两种疾病在发生、发展过程中相互影响,具有一定的关联性。因便秘引起的尿闭症的疾病有44种,其病因分类可归为10种,同样尿闭症的病因分类中因隆汇集、血盛、肿物、隆、赤巴、黄水等因素可引起便秘,同理因痞瘤、肾肿瘤、肾脓肿、膀胱的隆因素偏盛等四种疾病亦可引起少尿和大便不通。因此认为便秘是尿闭症的致病因素之一,同样在尿闭时也会出现便秘的症状,尿闭和便秘互为关联,相互影响,便秘时也可引起尿闭的症状。因此在治疗上通常采用同一种方法治疗这两种疾病。通过梳理和分析便秘与尿闭症这一内在关联性和治病机理为此类疾病的诊治提供了新思路、新方法,具有深入研究和推广应用的价值。The diseases related to constipation and anuresis in the Secret Medical Code of Tibetan Medicine and The Four Tantras are relatively common,but their classification is not clear,and the diagnosis is easy to be confused,because its pathogenesis is not only in the urinary system and intestinal tract,but also closely related to the five viscera,trauma,synthetic poisoning,etc.In clinical practice,the treatment is often based on the symptoms of constipation and urination,rather than the dialectic,analysis and treatment of the two diseases,thus affecting the clinical efficacy.This article discusses and analyzes the correlation and pathogenesis of constipation and anuresis based on the ancient Tibetan medical book The Four Tantras and other classics.Although constipation and anuresis are two different diseases,these two diseases influence each other and are mutually related.In addition,oliguria and fecal immobillity can also be caused by four kinds of disease such as edema,kidney tumor and abscess and excessive Lung factor of bladder.Therefor,constipation in considered to be one of the causative factors of anuresis,and constipation symptoms will also appear under anuresis.This paper specifically discusses how these two diseases are related.
