摘  要:目的探索苏州高新区35~65岁妇女下生殖道菌群的分布情况,并分析细菌性阴道病(bacterial vaginosis,BV)与下生殖道菌群的相关性。方法从苏州高新区2018年7月至2022年9月参与“两癌”筛查的女性中招募本项目受试者,并采集宫颈分泌物样本以及相关表型信息,从入组的女性中筛选出232名女性的样本进行分析,包括125名BV女性(BV组)和107名健康女性(对照组)。对分泌物样本进行宏基因组鸟枪法测序检测微生物菌群,并通过统计学方法分析宫颈微生物与BV的相关性。结果232名女性的阴道-宫颈微生物在种水平的研究提示阴道-宫颈菌群聚类成7种类型,BV导致惰性乳杆菌型(Type 1)减少,而加德纳菌型(Type 3)增加。进一步探索与BV相关的微生物标志物,发现加德纳菌(Gardnerella vaginalis)、人型支原体(Mycoplasma hominis)、疱疹病毒(Cyprinid herpesvirus 3)以及解脲支原体(Ureaplasma urealyticum)在BV组女性中明显富集。进一步按<50岁和≥50岁将女性分为中年组和老年组,发现不同年龄阶段,BV对菌群影响的程度不同。相对中年女性,老年女性阴道菌群结构受BV影响的程度更大。上述提到的BV相关标记物更容易在老年BV女性中富集。结论本研究通过宏基因组鸟枪法探索中老年女性下生殖道菌群谱图,发现BV对阴道菌群的影响会根据年龄轨迹发生变化,可为疾病的分年龄段诊断、治疗、评估预后提供一定的研究基础。Objective To explore the distribution of lower genital tract flora in women aged 35~65 in Suzhou High-tech Zone,and to analyze the correlation between bacterial vaginosis(BV)and lower genital tract flora.Methods Subjects of this project were recruited from women participating in the cervical and breast cancer screening in Suzhou High-tech Zone from July 2018 to September 2022,cervical secretion samples and related phenotypic information were collected,and 232 women's samples were screened for analysis from the enrolled women,including 125 BV women(BV group)and 107 healthy women(control group).Metagenomic shotgun sequencing was performed on secretion samples to detect microbial flora,and the correlation between cervical microorganisms and BV was statistically analyzed.Results The vaginal-cervical microbiome of 232 women was studied at the species level.The results showed that the vaginal-cervical microbiome was clustered into 7 types.BV resulted in a decrease in the inert Lactobacillus type(Type 1)and an increase in the Gardnerella type(Type 3).Further exploration of BV-related microbial markers revealed that Gardnerella vaginalis,Mycoplasma hominis,Cyprinid herpesvirus 3,and Ureaplasma urealyticum were enriched in BV group obviously.Women were further divided into middle-aged group and old-aged group according to <50 years old and≥50 years old.It was found that BV had different effects on flora at different ages.Compared to middle-aged group,women in old-aged group were more highly influenced by BV.The above-mentioned markers also accumulated more easily in old-aged group.Conclusion In this study,the metagenomic shotgun method was used to explore the lower genital tract flora of middle-aged and elderly women.It was found that the effect of BV on vaginal flora would change according to age trajectory,which provided a certain research basis for the diagnosis,treatment and prognosis of the disease by age.
