摘　　要：因脑卒中发病率高、死亡率高、致残率高、复发率高与并发症多等特点,患者出院后极易再次入院,严重影响其生存质量。科学规范的出院计划能够保证脑卒中患者获得连续的医疗护理照护,降低再入院率,改善患者结局,对降低脑卒中疾病负担具有重要意义。该文从出院计划的发展与内涵、护理模式以及在国内外脑卒中患者中的实施现状进行综述,并对实施脑卒中出院计划的机遇与挑战提出了建议,以期为科学规范的脑卒中疾病管理提供参考。Due to the high incidence rate,high mortality rate,high disability rate,high recurrence rate and many complications of stroke,patients are easily re-admitted after discharge,which seriously affects their quality of life.A scientific and standardized discharge plan can ensure continuous medical care for stroke patients,reduce readmission rates,and improve patient outcomes,which is of great significance for reducing the burden of stroke.This paper reviewes the development and connotation,the nursing model and the implementation status of the discharge planning among stroke patients at home and abroad,and makes recommendations on the opportunities and challenges of implementing the stroke discharge plan,in order to provide a reference for scientific and standardized stroke disease management.
