扶阳祛邪方在脑缺血再灌注损伤中的应用效果及增效减毒作用

摘　　要：目的研究扶阳祛邪方在脑缺血再灌注损伤患者中的应用效果及增效减毒作用。方法采取随机便利抽样法选取2019年10月—2020年9月医院脑缺血再灌注损伤患者68例为研究对象,按照单双号法将其随机分为观察组和对照组(各34例)。对照组予以常规西医治疗方案,观察组在对照组治疗基础上结合扶阳祛邪方,7 d为1个疗程、连续两个疗程。观察和比较治疗前后两组患者中医证候评分、神经功能缺损积分(National Institutes of health neurological deficit score,NIHSS)和B细胞淋巴瘤2(B-cell lymphoma-2,Bcl-2)、Bcl-2相关x蛋白(Bcl-2 associatea x protein,Bax)、半胱氨酸天冬氨酸蛋白酶-3(Cysteinyl aspartate specfic protease,Caspase-3)蛋白表达水平。结果观察组总有效率94.12%(32/34)高于对照组的82.35%(28/34),差异有统计学意义(P<0.05)。治疗后较治疗前,两组患者NIHSS评分和血清Bax、Caspase-3蛋白表达均下降,Bcl-2蛋白表达均升高,且治疗后观察组NIHSS评分和血清Bax、Caspase-3蛋白表达均低于对照组,Bcl-2蛋白表达高于对照组,差异有统计学意义(P<0.05)。结论扶阳祛邪方通过调控脑缺血再灌注损伤患者血清Caspase-3、Bcl-2与Bax蛋白表达水平而具有抗神经细胞凋亡等作用,可使患者收获良好的增效减毒效应,更大程度地保护脑神经功能。Objective To study the application effect of Fuyang Quxie Recipe(扶阳祛邪方)in patients with cerebral ischemia-reperfusion injury and its effect of enhancing efficacy and reducing toxicity.Methods A total of 68 patients with cere-bral ischemia-reperfusion injury in the hospital from October 2019 to September 2020 were selected by random convenience sampling method as the research objects,and they were randomly divided into the observation group and the control group(34 ca-ses in each)according to the odd-even method.The control group was given conventional western medicine treatment,and the observation group was given Fuyang Quxie Recipe on the basis of the treatment of the control group,7 d as one course of treat-ment,and there were two consecutive courses of treatment.The TCM syndrome score,neurological deficit score(NIHSS)and protein expression levels of B-cell lymphoma-2(Bcl-2),Bcl-2 associatea x protein(Bax)and cysteinyl aspartate specfic protease(Caspase-3)were observed and compared between the two groups before and after treatment.Results The total effective rate of TCM treatment in the observation group was 94.12%(32/34),higher than that in the control group[82.35%(28/34)],and the difference was statistically significant(P<0.05).After treatment,compared with those before treatment,the NIHSS scores and serum protein expression levels of Bax and Caspase-3 in the two groups were decreased,and the Bcl-2 protein ex-pression levels were increased.And after treatment,the NIHSS score and serum protein expression levels of Bax and Caspase-3 in the observation group were lower than those in the control group,and the Bcl-2 protein expression was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Fuyang Quye Recipe has the effect of anti-apoptosis by regulating the expressions of Caspase-3,Bcl-2 and Bax protein in the serum of patients with cerebral ischemia-reperfusion injury to protect the neural functions.
