摘 要:目的探讨马勃油膏对白兔慢性皮肤溃疡创面愈合情况及血管新生相关因子血管内皮生长因子(VEGF)、色素上皮源性因子(PEDF)的影响。方法24只新西兰大白兔随机分为空白对照组、模型组、阳性对照组及马勃油膏组,每组6只,以金黄色葡萄球菌制备慢性皮肤溃疡创面模型,造模后第2天起,马勃油膏组和阳性对照组白兔分别给予马勃油膏、重组牛碱性成纤维细胞生长因子凝胶治疗干预,空白对照组、模型组仅给予生理盐水清洁创面,每日1次,连续14 d。干预期间观察白兔的创面情况;在第3、7、14天时测定溃疡创面愈合率;免疫组化法、Western blot及RT-PCR检测溃疡创面组织中VEGF、PEDF表达及mRNA水平;ELISA法检测溃疡创面组织中白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平。结果马勃油膏组白兔的溃疡创面渗出较少,红肿消退较快,用药后创面颜色加深,愈合、结痂的速度较快。马勃油膏组在第3、7、14天时溃疡创面的愈合率均显著高于模型组(P<0.05),且创面愈合率随时间呈现为增高趋势。干预14 d后,免疫组化、Western-blot及RT-PCR结果可见,马勃油膏组白兔溃疡创面组织中的VEGF表达及mRNA水平均较模型组明显减少(P<0.05),PEDF表达及mRNA水平均较模型组明显增高(P<0.05)。ELISA结果可见,马勃油膏组白兔溃疡创面组织中的IL-6、TNF-α水平均较模型组明显降低(P<0.05)。以上各指标马勃油膏组和阳性对照组之间比较,差异均未见统计学意义(P>0.05)。结论马勃油膏能够明显促进白兔慢性皮肤溃疡创面的愈合,该治疗作用可能是通过促进溃疡创面血管新生、抑制炎症反应实现的,与阳性对照药重组牛碱性成纤维细胞生长因子凝胶的治疗效用相当。Objective To investigate the effects of Mabo Oil Ointment(马勃油膏)on wound healing and angiogenesis related factors vascular endothelial growth factor(VEGF)and pigment epithelium-derived factor(PEDF)in rabbits with chronic skin ulcer.Methods Twenty-four New Zealand white rabbits were randomly divided into blank control group,model group,positive control group and Mabo Oil Ointment group,with 6 rabbits in each group.The chronic skin ulcer wound model was prepared by Staphylococcus aureus.On the 2nd day after modeling,the rabbits in Mabo Oil Ointment group and positive control group were treated with Mabo Oil Ointment and recombinant bovine basic fibroblast growth factor gel respectively,while the rabbits in blank control group and model group were only treated with normal saline to clean the wound,once a day,for consecutive 14 d.The wound surface of rabbits was observed during the intervention,and the wound healing rate of ulcers was measured on day 3,7 and 14.The expression and mRNA levels of VEGF and PEDF in ulcer wound tissue were detected by immunohistochemistry,Western blot and RT-PCR.The levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in ulcer wound tissue were determined by ELISA.Results In Mabo Oil Ointment group,the ulcer wound exudation was less,redness and swelling subsided faster,the wound color deepened,and the healing and scab formed faster.The wound healing rate of Mabo Oil Ointment group on day 3,7 and 14 was significantly higher than that of model group(P<0.05),and the wound healing rate showed an increasing trend over time.After 14 days of intervention,immunohistochemistry,Western-blot and RT-PCR results showed that the ex-pressions of VEGF and mRNA in the wound tissue of white rabbit ulcer in Mabo Oil Ointment group were significantly decreased compared with those in the model group(P<0.05),and the expressions of PEDF and mRNA were significantly increased com-pared with those in the model group(P<0.05).ELISA results showed that the levels of IL-6 and TNF-αin rabbit ulcer w
