金橘胆胃丸治疗老年慢性胆囊炎伴胆结石临床疗效观察

摘　　要：目的观察金橘胆胃丸治疗老年慢性胆囊炎伴胆结石临床效果。方法研究纳入老年慢性胆囊炎伴胆结石合计102例(2019年3月—2022年3月收治),将本次纳入患者以随机数字表法分成对照组与观察组,每组51例,对照组接受熊去氧胆酸胶囊+头孢地尼胶囊治疗,观察组接受熊去氧胆酸胶囊及头孢地尼胶囊+金橘胆胃丸治疗,两组均治疗2个月。数据观察:疗效、中医证候(右上腹阵发性绞痛、胸闷纳呆、口苦口黏、恶心呕吐、目发黄等)积分变化及肝胆功能指标变化、结石直径及胆囊壁厚度变化、炎症因子指标变化、不良反应。结果观察组治疗总有效率(96.08%,49/51)比对照组(82.35%,42/51)更高,P<0.05;治疗前,两组患者中医证候(右上腹阵发性绞痛、胸闷纳呆、口苦口黏、恶心呕吐、目发黄等)积分、总胆固醇(TC)、总胆红素(TBiL)等肝胆功能指标、结石直径及胆囊壁厚度、白细胞介素6(IL-6)及肿瘤坏死因子等指标比较,P>0.05,治疗后各组患者中医证候(右上腹阵发性绞痛、胸闷纳呆、口苦口黏、恶心呕吐、目发黄等)积分、TC、TBiL、结石直径及胆囊壁厚度、IL-6、肿瘤坏死因子等指标均改善,观察组治疗后中医证候(右上腹阵发性绞痛、胸闷纳呆、口苦口黏、恶心呕吐、目发黄等)积分、TC、TBiL、结石直径及胆囊壁厚度、IL-6、肿瘤坏死因子等指标均优于对照组,P<0.05;两组不良反应均较少,P>0.05。结论金橘胆胃丸治疗老年慢性胆囊炎伴胆结石临床效果显著,可促进结石消溶及排出,改善患者肝胆功能,安全可靠,值得应用。Objective To observe the clinical effect of Jinju Danwei Pill(金橘胆胃丸)on chronic cholecystitis with gallstones in the elderly.Methods A total of 102 elderly patients with chronic cholecystitis complicated with gallstones(admitted from March 2019 to March 2022)were included in this study.The patients were divided into control group and observation group by the ran-dom number table method,with 51 patients in each group.The control group received ursodeoxycholic acid tablets+cefdinir cap-sules.The observation group received ursodeoxycholic acid tablets and cefdinir capsule+Jinju Danwei Pill.Both groups were treated for 2 months.The curative effect,the scores of TCM syndromes(parparal colic of right upper abdomen,chest tightness,bitter and sticky mouth,nausea and vomiting,yellow eyes,etc.),changes in hepatobiliary function indexes,changes in stone di-ameter and gallbladder wall thickness,changes in inflammatory factors indexes and adverse reactions of two groups were com-pared.Results The total effective rate of the observation group(96.08%,49/51)was higher than that(82.35%,42/51)of con-trol group(P<0.05).Before treatment,the scores of TCM syndromes(parparal colic of right upper abdomen,chest tightness,bit-ter and sticky mouth,nausea and vomiting,yellow eyes,etc.),total cholesterol(TC),total bilirubin(TBiL)and other liver func-tion indexes,stone diameter and thickness of gallbladder wall,interleukins 6(IL-6)and tumor necrosis factor of two groups had no significance(P>0.05).After treatment,the scores of TCM syndromes(parparal colic of right upper abdomen,chest tightness,bitter and sticky mouth,nausea and vomiting,yellow eyes,etc.),the levels of TC and TBiL,stone diameter and gallbladder wall thickness,the levels of IL-6,tumor necrosis factor and other indicators were improved in each group.The indexes above in the observation group were better than those in the control group(P<0.05).There were few adverse reactions in both groups(P>0.05).Conclusion Jinju Danwei Pill has a significant clinical effect on the tr
