糖肾消肿方治疗糖尿病肾病合并水肿临床观察

摘　　要：目的:观察糖肾消肿方治疗糖尿病肾病Ⅳ期合并水肿患者的临床效果。方法:将74例2型糖尿病肾病合并水肿患者随机分为治疗组36例和对照组38例。两组均予西医基础治疗,对照组予托拉噻米片,治疗组予糖肾消肿方治疗,疗程均为2周,观察治疗前后两组水肿评分、中医证候评分以及尿微量白蛋白/尿肌酐、血白蛋白、血肌酐、血尿素、血尿酸、血Cys-C水平。结果:水肿评分评价治疗组有效率为91.7%,对照组为71.1%,差异有统计学意义(P﹤0.05);治疗组中医证候评价总有效率86.1%,对照组总有效率13.2%,差异有统计学意义(P﹤0.01);两组整体疗效比较治疗组总有效率为91.6%,对照组总有效率为36.8%,差异有统计学意义(P﹤0.01);同时治疗组尿微量蛋白/尿肌酐、血白蛋白、血尿酸、血尿素、血Cys-C水平均较对照组改善明显(P﹤0.01,P﹤0.05)。结论:糖肾消肿方能更明显地改善水肿,改善患者全身症状,临床疗效较好。Objective:To observe the clinical efficacy of Tangshen Xiaozhong formula on 5th stage of diabetic nephropathy complicated with edema.Methods:74 cases of patients with type 2 diabetic nephropathy complicated with edema were randomly divided into treatment group(36 cases)and control group(38 cases).Both groups were treated with basic western medicine.In addition,the control group was treated with torasemide tablets,while the treatment group was treated with Tangshen Xiaozhong formula.The course of treatment was 2 weeks.Edema score,TCM syndrome score as well as the levels of urinary microalbumin/urine creatinine,serum albumin,serum creatinine,blood urea,blood uric acid and serum Cys-C were observed in two groups before and after treatment.Results:In the evaluation of edema score,the total effective rate was 91.7%in the treatment group and 71.1%in the control group respectively,and the difference was statistically significant(P﹤0.05).In the evaluation of TCM syndrome,the total effective rate was 86.1%in the treatment group and 13.2%in the control group respectively,and the difference was statistically significant(P﹤0.01).In the comparison of overall efficacy between two groups,the total effective rate was 91.6%in the treatment group and 36.8%in the control group respectively,and the difference was statistically significant(P﹤0.01).In addition,the levels of urinary microalbumin/urine creatinine,serum albumin,blood uric acid,blood urea and serum Cys-C of treatment group were significantly improved compared with those of control group(P﹤0.01,P﹤0.05).Conclusion:Tangshen Xiaozhong formula can obviously improve edema,improve patients'systemic symptoms,and has a better clinical efficacy.In addition,it can also reduce the levels of urinary microalbumin/urine creatinine,blood uric acid,blood urea and serum Cys-C,and increase the level of serum albumin.
